The DMR for the referenced facility for the above -referenced. month was submitted via the North
Carolina e-DMR system. Please find attached the signed hardcopies (one original and one copy) of the
e-DMR submittal. Note that during the month of August, the referenced facility was connected to the
town of Taylorsville sanitary sewer ' system. There was ` no flow through the Shurtape wastewater
treatment plant after 9 August 2018. Shurtape expect to submit a request to rescind Permit NCO072664 in
the near future.

Beginning on August 9, 2018, the Stony Point facility's sanitary sewage flow was connected to the town of Taylorsville sanitary sewer line, and flow to the on -site wastewater
