Please accept this letter as Shurtape's written notification of the No. 2 fuel oil spill that
occurred on 5 August 2002. This letter is also in follow up'to the telephone notification made to
you on 5 August 2002.
At approximately 8 AM on 5 August 2002, employees at Shurtape's Stony Point facility
noted a leak of fuel oil in the concrete containment basin. Further investigation by Shurtape
personnel found the leakage to be coming from a supply line inside of a carrier pipe. The supply
line enters the stand by boiler through the carrier pipe under said boiler room. Upon opening the
sealed end of the carrier pipe to initiate inspection of the supply line, approximately 50 gallons of
No. 2 fuel oil was released into the concrete containment area. Approximately 25 gallons was
immediately pumped back into a 55-gallon drum. The remaining volume was spread out over an
area approximately 40 feet by 40 feet. Spill clean up operations were immediately initiated with
_ the initial volume being removed along with absorbent materials on 5 August 2002. Due to the
rough surface of the concrete and the presence of accumulated soil materials on the concrete,
additional absorbent was applied, covered with plastic, and held in place with 75-pound blocks of
rubber to prevent any penetration by moisture. The absorbent material was allowed to remain in
place until 9 August 2002 to facilitate removal of any further residual. On 12 August 2002, the
area was cleaned using a pressure washer and hydrocarbon converting soap. All materials
associated with the clean up operations have been properly manifested and shipped for disposal.
The area of the spill has been completely cleaned up with no further indications of remaining fuel
oil.
Based upon the above clean up efforts, Shurtape does not believe that a release to the
environment occurred. Shurtape is currently in the process of locating the necessary equipment
to visually inspect the carrier pipe from the -point of containment to the spare boiler room to
insure that a release to the environment did not occur.

Technologies, Inc. Please be advised that this report has been prepared in accordance with the format
suggested in Kerr T. Stevens' 19 October 1999 letter.

d. Description of treatment process: Shurtape Technologies operates a 10,000 gpd extended
aeration type WWTP to treat domestic wastewater generated by Shurtape's Stony Point
Tape Plant. The extended aeration plant consists of an inlet bar screen, aeration basin,
hopper type clarifier w/ a multiple v-notch type weir, tablet -type chlorination, and an
aerated sludge holding tank. Discharge of the treated wastewater is into Third Creek in
the Yadkin -Pee Dee River Basin. To optimize the operation of the WWTP, Shurtape has
implemented in-house testing of MLSS in the aeration basin and grab TSS in the effluent.
Il. Performance
a. Description of overall 12-month performance:
The previous twelve months performance was reviewed with the,ORC, Mr. Travis
Brannon, Certificate No. 25140. Review of the system performance consisted of
reviewing the operator's logbook, process control equipment calibration logbooks, sludge
removal records, and DMR's from July 2001 through June 2002. During this time
period, no effluent violations occurred. During this time period all process control
monitoring equipment was found to have meet calibration checks; all buffer solutions
used for pH meter calibrations were valid; and all sampling results have attached chain of
custody forms. The operator logbooks verified that the system had been attended as
required with maintenance activities performed.
b. During the time period of July 2001 through June 2002 no effluent violations occurred.
C. Description of any known environmental impacts of violations:
N/A — No known violations of effluent limitations.
d. Description of corrective actions taken to address violations or deficiencies: N/A

Shurtape has met the requirements to conduct analysis of field parameters on site through
Lab Certification Procedures.

Confirmation results received in June 2002.
III. Notification
Shurtape's WWTP serves the manufacturing facility at Stony Point. There are no other users
connected to the system. Therefore, Shurtape is the only user of the system. A• copy of this report
is sent to the Vice President of Manufacturing and the Executive Vice President.

This request was originally sent to your attention on 18 July 2000; however, to date, Shurtape has
not received a response. As our current ORC is nearing retirement it is imperative that Shurtape make this
