5. Directions to Site: From the intersection of NC Hwy.. 90 and SR 1628 (Hunter Bridge
Road) in eastern Alexander County near the community of Stony Point, travel west on
NC Hwy. 90 approximately 0.15 mile. The Shurtape Technologies plant is located on the
left side of the Hwy. 90.

for expansion and upgrading at the existing site.

well removed from any flood plain.
9. Location of nearest dwelling: Several dwellings are located approximately 150 feet from
the WWTP.

C. Describe receiving stream features and pertinent downstream uses: The discharge
enters a small stream (6 - 8 feet wide x 2 - 6 inches deep) that has a 7Q 10 flow of
0.17 cfs. Stream has moderate gradient and a sandy bottom. General "C"
classification uses downstream.
PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS
1. Type of wastewater: 100% Domestic

a. Volume of Wastewater: 0.010 MGD (Design Capacity)
b. Types and quantities of industrial wastewater: There is no industrial wastewater
generated at this facility.
C. Prevalent toxic constituents in wastewater: Chlorine is added for disinfection.
d. Pretreatment Program (POTWs only): N/A
3. Type of treatment (specify. whether proposed or existing): Existing treatment consists of
a 10,000 gpd extended aeration package plant, which includes a bar screen followed by an
aeration tank, a clarifier, a sludge holding tank, and chlorination with a tablet type
chlorinator. There are no proposed WWT facilities at this time.
4. Sludge handling and disposal scheme: Sludge is pumped by Lentz Septic Service and is
transported for disposal to the City of Statesville Fourth Creek WW"IP.
5. Treatment plant classification: No change from current rating (Class II).

PART III - OTHER PERTINENT INFORMATION
Is this facility being constructed with Construction Grant Funds (municipals only)?
Public monies were not used in the construction of this facility.
2. Special monitoring requests: None at this time.
3. Additional effluent limits requests: It is anticipated that a residual chlorine limit will be
included in the permit upon renewal (see Part IV).

4. Alternative Analysis Evaluation
a. Spray Irrigation: Insufficient area, poor soils. Adjacent property is either
unavailable or is restricted by topography and residences for on -site disposal.
b. Connect to regional sewer system: There is no municipal or private sewer
collection system within several miles of this facility.
C. Subsurface: One disposal option that is to be considered by the permittee is the
use of an abandoned septic system that this facility once used many years ago.
The septic system lies adjacent to the Shurtape plant, and is thought by plant staff
_to have sufficient capacity to handle all or at least a portion of the current
wastewater flow (currently less than 3000 gpd). Depending on the viability of this
option, Shurtape is also going to consider the option of a conjunctive NPDES
permit, which would allow them to utilize both the septic system and the current
NPDES discharge. Shurtape had also considered the possibility of wastewater
reuse, however, the current facilities will not comply with the Division's reuse
guidelines.
PART IV - EVALUATION AND RECOMMENDATIONS
The permittee, Shurtape Technologies, Inc., has applied for renewal of the subject Permit.
There have been no changes to the existing WWT facility or to the permit since the permit was
last reissued.
The most recent inspection report indicates that the facility is being well maintained and
generally able to meet existing effluent limits. The site inspection conducted for the purposes of
this permit renewal confirmed the findings of the inspection report.
Please note that the map contained in the current permit indicating the point of discharge
and the corresponding latitude coordinates is inaccurate. A revised USGS map is attached
showing the correct point of discharge. The revised latitude coordinates can be found in Part I,
No. 6 of this report.
It is recommended that the ermit be renewed as requested.

delegated signature authority for all environmental permitting and reporting requirements

➢ Air Permit applications, notifications, reports, and compliance certifications as required
under the Clean Air Act;
➢ Wastewater Discharge permit applications, reports, and compliance certifications as
required under the Clean Water Act;
➢ Reporting requirement for TIER II and TRI under EPCRA; .

Dear Mr. Weaver:
Shurtape is requesting that the Division of Water Quality renew the subject
NPDES permit that expires on 31 July 2004. Please find attached the, completed NPDES
Application for Permit Renewal (Short Form D) and a delegation of signature authority
letter. .
Should the Division determine that an effluent .limitation for Chlorine is
appropriate for inclusion with renewal of the permit, Shurtape requests that a compliance
schedule be incorporated into the new permit requiring compliance the new limitation
approximately 18 months from the date the permit would become effective. Per our
earlier telephone conversation on this date, Shurtape understands that this is the
Division's current policy. Should a schedule different from this be proposed by the
Division Shurtape requests to be contacted immediately.
