Please be advised that this report has been prepared.in accordance with the format suggested in Kerr T. Stevens' 19 October 1999 letter. I. General Information a. Description of treatment process: Shurtape Technologies operates a 10,000 gpd extended aeration type WWTP to treat domestic wastewater generated by Shurtape's Stony Point Tape Plant. The extended aeration plant consists 'of an inlet bar screen, aeration basin, hopper type clarifier w/ a multiple v-notch type weir, tablet -type chlorination, tablet de -chlorination, and an aerated sludge holding tank. Discharge of the treated wastewater is into Third Creek in the Yadkin -Pee Dee River Basin. To optimize the operation of the WWTP, Shurtape conducts process testing of MLSS, MLVSS, and settleability in the aeration basin. II. Performance a. Description of overall 12-month performance: The previous twelve months performance was reviewed with the ORC, Mr. Travis Brannon, Certificate No. 990808. Review of the system performance consisted of reviewing the operator's logbook, process control equipment calibration logbooks, sludge removal Shurtape Technologies, LLC PO Box 1530 Hickory, NC 28603 www.shurtape.com Annual System Performance Report 28 January 2009 Page 2 of 3 records, and DMR's from January 2008 through December 2008. During this time period, no effluent violations occurred. During this time period, all process control monitoring equipment was found to have met calibration checks, with exception of two quality control (QC) checks explained in (b) below; all buffer solutions used for pH meter calibrations were valid; and all sampling results have attached chain of custody forms. The operator logbooks verified that the system had been attended as required with maintenance activities performed. During 2008, there were no events noted in the logbook that would cause the facility to operate in a non -compliant manner. The facility operated in compliance. b. During the time period of January.2008 through December 2008 no effluent violations occurred. Upon review of the DMRs submitted for, the reporting period, and comparison with calibration records, it was found that low-level_ chlorine results were reported for two days (03 January 2008 and 15 October 2008) in which the quality control procedures were not met due to'a non - check of the secondary standard for low-level chlorine. All previous and post QC checks were satisfactory, and the equipment was within calibration dates; therefore, posted data should be accurate. The DMRs for January and October 2008 have been amended to footnote the chlorine results for those two days, and the DMRs were resubmitted. Additionally, the following were identified: • On 28 July 2008, the effluent temperature was inadvertently reported as 24.1 degrees C, when the actual temperature was 25.1 degrees C. • On 22 October 2008, the downstream DO was inadvertently reported as 10.3 mg/L, when the actual downstream DO was 10.4 mg/L. The DMRs for July and October 2008 have been corrected and resubmitted. C. Description of any known environmental impacts of violations: N/A — No known violations of effluent limitations. d. Description of corrective actions taken to address violations or deficiencies: . No known violations existed for the reporting period. Section (b) above describes four events that were identified: On two occasions, low-level chlorine results were reported when there . was not corresponding QC check of the low-level standard. The equipment was within calibration dates, and all previous and post calibration checks were accurate, therefore the reported results should be correct. The DMRs for those two dates have been amended to note this and have been resubmitted. • On one occasion, the effluent temperature was inadvertently reported one degree lower than the actual reading. The .DMR has been corrected and resubmitted. Annual System Performance Report 28 January 2009 Page 3 of 3 • On one occasion, the downstream DO was inadvertently reported 0.1 . mg/L lower than the actual reading. The DMR has been corrected and resubmitted. e. Shurtape has met the requirements to conduct analysis of field parameters on site through Lab Certification Procedures. f Lab Certification No. 5087 EPA'Lab ID No. NCO1311 Proficiency testing was performed during August 2008 and approval report issued 16 September 2008. g. Notification: Shurtape's WWTP serves the manufacturing facility at Stony __-----V.-----Puirit. Where are ;no other users connected, -to the system.-Therefore-Shurtape is the only user of the system. A copy of this report is sent.to the Vice President of Manufacturing and the Executive Vice President. III. Certification I certify, under penalty of law, -that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Summary of ORC Log Book Review: Logbook is up to date with appropriate notations of actions by ORC and back-up ORC. Logbook apears to contain all operational information required to demonstrate compliance. Dates and times appear to be noted correctly. Information in logbook corresponds to that recorded on the DMRs submitted throughout CY2008, with the exception of the following: - On January 3, 2008, low level chlorine was reported without a corresponding QC check of the low-level 'standard; - On July 28, 2008, effluent temperature was reported as 24.1 degrees C, but actual temperature was 25.1 degrees C; 'On October 15, 2008, low level chlorine was reported without a corresponding QC check of the low-level standard; and, - On October 22, 2008, downstream DO was reported as 10.3 mg/L while the actual reading was 10.4 mg/L. - NOTE that for the low level chlorine readings, all prior and post QC checks were satisfactory, and the equipment was within calibration dates, so the readings should be accurate. DMRs were corrected and resubmitted addressing each of the above events. Summary or tteview: Annual Performance_ Test for Field Parameters: pH: Method 4500HB & TRC: Method 4500 CIG Corning pH & YSI DO meters both have correction factors noted on meters (current factor is "0"). Modified DMRs were submitted to address the following: - Noting that the January 3, 2008 and October 15, 2008 chlorine readings were reported without a corresponding QC check of the low-level chlorine standard; however, the instrument was within calibration dates, and prior and post QC checks were - Correcting the July 28, 2008 temperature reading and October 22,2008 downstream DO readings. Initial review was performed on January 23, 2009. All previous and post QC checks were satisfactory, and the equipment was within calibration dates, therefore the reported concentrations should be accurate. • On 15 October 2008, low-level chlorine.was reported, however, all of the quality control (QQ procedures were not met because of a non -check of the low level chlorine standard. All previous and post QC checks were satisfactory, and the equipment was within calibration dates, therefore the reported concentrations should be accurate. • On 28 July 2008, effluent temperature was inadvertently reported as 24.1°C, where as'on-site records indicate that the actual temperature was 25.1°C. • On 22 October 2008, downstream DO was inadvertently reported as 10.3 mg/L, where as on -site records indicate that the actual DO was 10.4 mg/L. Please find attached the original and one copy of the amended DMRs for January 2008, July 2008, ,and October 2008. During the referenced months, the wastewater treatment plant operated in compliance with all effluent limitations specified in the subject permit.
