The Shuford Mills plant is located. on the left side of the road. 6. Discharge Point(s), List for all discharge points: Latitude: 350 51' 36" Longitude: 810 03' 23" Attach a USGS map extract and indicate treatment facility site and discharge point on map. Topography (relationship to flood plain included): Flat to moderate slopes; the WWTP is not located within the 100 year flood plain. 9. Location of nearest dwelling: Several dwellings are located .approximately 150 feet from the WWTP. 10. Receiving stream or affected surface waters: Third Creek a. Classification: C b. Stream has moderate stream gradient with a sandy bottom. General "C classification uses downstream. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. Type of treatment (specify whether proposed or existing): Existing treatment consists of a 10,000 gpd extended aeration package plant, which includes an aeration tank, clarifier, sludge holding tank and chlorination with a tablet type chlorinator. 5. Sludge handling and disposal scheme: Sludge is pumped by Lentz Septic Service and is land applied under a DHS septage disposal permit (NCS00147). The facility will be notified that the septage permit does not cover land application of activated -sludge. The facility should be given 60 days from Permit renewal to submit plans for sludge disposal. 6. Treatment plant classification (attach completed rating sheet): Class II 7. Other: N/A PART IV - EVALUATION AND RECOMMENDATIONS The permittee, Shuford Mills, Inc.,.has applied for Permit renewal. The facility appears well maintained and able to meet existing effluent limits. The facility should be required to submit a sludge disposal plan within 60. days of permit renewal since the.existing disposal plan is not adequate. The site shall beloperated and maintained in such a manner as to prevent migration of wastes off of the designated waste receiving site. The installation of groundwater monitoring wells may be.required. I. A suitable vegetative cover shall be maintained in accordance with the crop management plan approved by the Divisionvegetative cover over active sites or active portions of sites shall be kept mowed to a height of 6 to 10 inches. 4.` The issuance of.this permit does not preclude the Permittee from complying with any and all statutes, .rules, regulations, or ordinances which may be imposed by other government agencies (l.ocal,-state and federal) which have jurisdiction. 5. This permit may be modified or reissued to incorporate any conditions, limitations and monitoring requirements the Division of Solid Waste Management deems necessary to. adequately protect the environment and public health, 6. Septage shall be lime stabilized to a pH.of 12 prior to soil application, and_ incorporated into the.soil'as nee.ded'to maintain the site in a sanitary condition... A - septage detention -system should be set up on site to facilitate mixing and to be certain that the.septage has adequate time to. react with the lime. At leafdt 50 pounds. of hydrated lime shall be mixed with each 1000 gallons of septage, more may be necessary. Septage shall not be applied.during periods of,high soil moisture. 7. Field #1 contains approxiinately 23 acres that are available.for septage disposal. The maximum.annual .application rate shall be 30,�000 gallons per acre per year, if an approved crop management plan is implemented. 'For dedicated sites, the maximum annual application rate shall be 15,000 gallons per acre per year. These application rates assume equal septage distribution, on an annual basis, over the permitted area. 8. An approved septage detention system with a minimum design capacity of 15,000 gallons shall be available prior to operation of this site. This capacity shall equal the average volume of septage that,is pumped in one week, unless an approved wastewater treatment plant is available for use during periods of .adverse weather. The storage capacity may be adjusted if it is demonstrated during the operation of the site that this volume of storage is inappropriate. A written plan must be submitted to the Septage Management Branch within 30 days from the issuance of this permit. It should -be possible to obtain a plan through the.county Soil Conservation Service office. 10.0nly those -areas designated on the attached site map(s) shall be utilized for septage.disposal. Each load of septage discharged at the site shall be distributed evenly over as large an area as possible from a moving vehicle. 11.This permit shall become voidable unless the land application activities.are carried out in accordance with the conditions of this permit and in the manner approved by this Division. 12.Prior to any transfer of this land, a notice shall be given to.the new owner that gives full details of the .materials applied or incorporated at this site. 13.This permit -shall be renewed five years from the date of issuance. Modifications, where necessary, shall be made in accordance with the rules in effect at the.time of renewal. An application for permit renewal shall be submitted at least ninety (90) days prior to t'he permit renewal date. A written plan must be submitted to the Septage Management Branch within 30 days from the issuance of this permit. It should be possible to obtain a plan through the.county Soil Conservation Service office. 10.Only those areas'designated.on the attached site maps) ,shall be utilized for septage disposal:. Each load of septage discharged at the site shall be distributed evenly over as large an area as possible from a moving vehicle. ll.This permit shall become voidable unless the land. application activ.ities'are carried out in accordance with the cond.itions"of this permit and in the manner approved. by .this Division. 12.Prior.to any transfer of this land, a notice shall be .given to the new owner that gives full details of the materials applied or incorporated at this site. 13.This permit shall be renewed five years from the date of issuance. Modifications,'where necessary, shall be made in accordance with the rules in effect at the.time of. renewal. An application for permit. renewal shall be' submitted at least ninety (90) days prior to the permit renewal date. This permit is non -transferable. 14.Any.duly authorized officer, employee, or representative. of the Division of Solid Waste Management may, upon presentation of credentials, enter and inspect any property, -premises, or place on'or related to the disposal site and facility at any reasonable time for the purpose of determining compliance with this, permit;. may inspect or copy any records that must be kept under the conditions of this permit; or may obtain samples of .groundwater, surface water, or leachate. 15.The areas which can be used sept age shall be maintained existing wells, residences, places of public assembly. disposed of within 50 feet 200 feet of a pond. 16.Site location:. latitude longitude for land application of at least 500 feet from places of business, or.
