conducted at the subject facility on December 5, 2013, by Ms. Marcia Allocco and Mr. Wes Bell of this

Comment: ' The ORC and staff must ensure that they conduct and document aliquot
verifications (100-mL per aliquot minimum) on the composite samplers (influent and
effluent) on a periodic basis. Any maintenance or repairs performed on the samplers
e.g. tubing replacement should also be documented. The influent composite sampler
was collecting 150 mL per aliquot when tested; the thermometer within the refrigerated
sampler was 2 degrees C.

verifications (100-mL per aliquot minimum) on the composite samplers (influent and

Comment: On -site analyses (pH and TRC) are performed under field laboratory
certification #5062 (Town of Taylorsville). Water Tech Labs, Inc. (Certification #50)
performs all remaining permit -required effluent analyses and Research and Analytical
Laboratories has been contracted for WET testing. The field instrumentation used on
site appeared to be properly calibrated and documented; daily calibration/verification
data is entered in the ORC log book. The field instrumentation is calibrated annually by
Water Tech labs, Inc. All instrumentation was calibrated in January 2013.

Comment: The permittee's records were organized and well maintained and records
requested during the inspection were readily available. DMRs were reviewed for the
period October 2012 through September 2013. No limit violations were reported and all
monitoring frequencies were correct. The submitted DMRs have noted Water Tech
Labs, Inc. (laboratory certification #50)'as the certified laboratory. used for permit
required analytical support. Please add the Town of•Taylorsville and Research and
Analytical Laboratories as certified laboratories on future DMR submissions. The
permittee has been using parameter code 00094 to report effluent conductivity
concentrations; the correct parameter code is 000095. The 2012 annual report was
reviewed on site; sent to users on 7/30/13.
