Directions to site: From the jct. of SR 1637 (Smith Farm Rd. and Hwy. 90 near the Iredell/Alexander County line, travel northwest on Hwy. 90 Z 0.5 mile. The Shur -tape Technologies plant will be on the left side of Hwy. 90. 6.. Discharge point(s), list for all discharge points: outfall 001 Latitude: 35 ° 5l' 55" Longitude: 81 ° 03' 10" Attach a USGS Map Extract and indicate treatment plant site and discharge point on map. USGS Quad No.: D 14 SE 7. Site size and, expansion area consistent with application: Yes. Sufficient area exists for the construction of WWT facilities, if necessary. 8. Topography (relationship to flood plain included): Gently rolling, 2-5% slopes. The site is not located in a flood plain. Page Two 9. Location of nearest dwelling: Approx. 200f feet from the plant site. 10. Receiving stream or affected surface waters: U.T. to Third Creek" a. Classification: C b. River basin and subbasin no.: Yadkin 030706 C. Describe receiving stream features and pertinent downstream uses: Discharges from outfall 001 enters a dry ditch, which eventually drain to Third Creek. The ditch pass in close proximity to several off -site residences. *Applicant proposes to connect the waste stream covered under the subject Certificate of Coverage (COC) to an existing outfall covered under NPDES permit no: NC0072664. Once relocated, the wastewater permitted under this COC will discharge directly to Third Creek. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS ►A 3. a. Volume of wastewater: outfall 001 - 0.0154 b. What is the current permitted capacity: There is no permitted capacity C. Actual treatment capacity of current facility (current design capacity): outfall 001 consists of cooling tower (1600 gpd) and air washers (12,700 gpd). Boiler blowdown wastewater previously permitted in this COC as outfall 002 has been connected to an existing package W WTP_ covered under NPDES Permit No. NC0072664. d. Date(s) and construction. activities allowed by previous ATCs issued in the previous two years: There have been no ATCs issued to this facility in the past two years. e. Description of existing or substantially constructed WWT facilities: There are no existing WWT facilities. f. Description of proposed WWT facilities: There are no proposed WWT facilities. g. Possible toxic impacts to surface waters: Biocides are added to the current waste streams tributary to outfall 001. Residual handling and utilization/disposal scheme: There are no residuals generated. Treatment plant. classification: This facility does not meet -the minimum requirements for a Class I rating. 4. Is this facility being constructed with Construction Grant Funds or. are any public monies involved (municipals only)? Public monies were not used in the construction of this facility. 2. Special monitoring or limitations (including toxicity) requests: None at this time. 3. Important SOC or Compliance Schedule dates: This facility is neither under an SOC nor . is one being proposed at this time. 4. Alternative analysis evaluation a. Spray Irrigation: Insufficient area. b. Connect to regional sewer system: None available to serve the site. c. Subsurface: Insufficient area. PART IV - EVALUATION AND RECOMMENDATIONS The applicant requests that the Certificate of Coverage (COC) be reissued for the discharges of boiler blowd6wn and non -contact cooling water at the subject facility. Changes have occurred since the last COC was issued (see paragraph below and Part 11, No. 1(c) above). During the term,of the current COC, a previously permitted outfall (002) was connected to an existing outfall serving the facility's package WWTP. This outfall is authorized by NPDES Permit No. NCO072664 and discharges directly to Third Creek. The applicant is planning to connect the remaining outfall authorized under this COC.to the outfall permitted under , NCO072664 at a point below all current treatment process, but where sampling can be conducted prior to commingling. Pending a final review and approval by NPDES Unit, it is recommended that the COC be reissued as requested.
