Based on your request to renew the subject permit (received September 22, 2014), the Division of
Water Resources (DWR or the Division) hereby transmits a renewal draft for your review and
comment. Please review this draft carefully to ensure your understanding of the limits and monitoring
conditions; and to correct errors, if any.
Concurrent with this notification, the Division will solicit public comment on this draft by publishing a
notice in newspapers having circulation in the general Alexander County area, as required by the
NPDES Program. Please provide your written comments, if any, to DENR / DWR / NPDES Program
no later April,28, 2015 (approximately 30 days after receiving this document). Your written comments
are welcome, but are not mandatory.
Changes for Renewal. We understand that you have upgraded your WWTP headworks during the last
permit cycle, in accord with the Division's Authorization to Construct (ATC), dated November 19,
2010. We have therefore made minimal changes to your permit by updating your facility description and
site map. The revised map reflects your updated receiving -stream segment fiom the previous 11-69-(0.5)
to the segment 11-69-(5.5), classified WS-IV on the Lower Little River.
We have also added monitoring parameter codes for your convenience [see permit Section A. (1)].
However, please note that a new reporting program, electronic reporting of Discharge Monitoring
Reports (DMRs), is now required within 270 days of the permit effective date.
Implementing Electronic Discharge Monitoring Reports eDMRs). Please be advised that the Division
has implemented an electronic Discharge Monitoring Report (eDMR) program, in accord with pending
requirements by the Environmental Protection Agency (EPA). We have included the details required to
implement the eDMR program in this permit [see permit Section A. (3.)].
Proposed federal regulations require electronic submittal of all DMRs and specify that,. if North
Carolina does not establish a program to receive such submittals, Permittees must then submit eDMRs

A. (2.) CHRONIC TOXICITY (QUARTERLY) - LIMITED
[G. S. 143-215.1(b)]
The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to
Ceriodaphnia dubia at an effluent concentration of 8.2%.
The permit holder shall perform at a minimum, quarterly monitoring using test procedures outlined in the "North Carolina
Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent versions or "North Carolina
Phase 11 Chronic Whole Effluent Toxicity Vest Procedure" (Revised -February 1998) or subsequent versions. The tests will
be performed during the months of January, April, July and October.. Effluent sampling for this testing shall be
performed at the NPDES permitted final effluent discharge below all treatment processes.
If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit
limit, then multiple -concentration testing shall be performed at a minimum, i�'b ch of the lhvo following months as
described in "North Carolina Phase H Chronic Whole Effluent Toxicity T �t R .deedure" (Revised -February 1998)
or subsequent versions.

the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the
Permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the facility name,
permit number, pipe number, county, and the month/year of the report with the notation of "No Flow" in the comment area
of the form. The report shall be submitted to the Environmental Sciences Section at the address cited above.
Should the Permittee fail to monitor during a month in which toxicity monitoring is required, monitoring will be required
during the following month.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water
Resources indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include
alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism survival,
minimum control organism reproduction, and appropriate environmental controls, shall constitute an invalid test and will
require immediate follow-up testing to be completed no later than the last day of the month following the month of the
initial monitoring.

If a Permittee is unable to use the eDMR system due to a demonstrated hardship, or due to the
facility being physically located in an area where less than 10 percent of the households have
broadband access, then a temporary waiver from the NPDES electronic reporting requirements may
be granted, and discharge monitoring data may be submitted on paper DMR forms (MR 1, 1.1, 2, 3),
or alternative forms approved by the Director. Duplicate signed copies shall be submitted to the
mailing address above.
Requests for temporary waivers fiom the NPDES electronic reporting requirements must be
submitted in writing to the Division for written approval at least sixty (60) days prior to the date the
facility would be required under this permit to begin using eDMR. Temporary waivers shall be
valid for twelve (12) months),and shall thereupon expire. At such time, DMRs shall be submitted

electronically to the Division unless the Permittee re -applies for and is granted a new temporary
waiver by the Division.

Regardless of the submission method, the first DMR is due on the last day of the month following
the issuance of the permit or in the case of a new facility, on the last day of the month following the
commencement of discharge.

INTRODUCTION
The Town of Taylorsville requires a permit from the National Pollutant Discharge Elimination
System (NPDES) to operate its wastewater treatment plant and dispose heated wastewater to the
surface waters of the state. The permittee's 5-year NPDES permit expires March 31, 2015 and the
Town has requested renewal from the Division of Water Resources (the Division), This Fact
Sheet summarizes background information and rationale used by the Division's NPDES Unit to
determine permit limits and monitoring conditions.

to renew (Standard Form 2A), received 22Sep2014. Although BIMS indicate a complex
wastestream [-70 Domestic / 30% industrial], Taylorsville WWTP does not have a pretreatment

Changes to Facility Description
The Town of Taylorsville WWTP received an Authorization to Construct (ATC) from the
Division (19Nov2010) to install new components at the headworks. These facilities were
confirmed completed by Engineer's Certification received. from Benjamin B. Thomas, PE
(10kn2013). These improvements have been added to the facility description for renewal (see
Supplement to Permit Cover Sheet) as:

Revised Receiving -Stream Segment. Previous permits have incorrectly included this outfall
within receiving -stream segment 11-69-(0.5), a Class C waterbody. This renewal relocates the
outfall to the correct segment immediately downstream: Segment 11-69-(5.5), Classified WS-IV
(see Map, Permit Supplement to Cover Sheet, and the attached documentation).
Receiving -Stream Conditions. Start-up Level-B modeling suggested secondary limits as adequate to
protect the receiving stream; no instream monitoring recommended. Instream monitoring is not
recommended for renewal. This facility discharges to receiving waters designated as the Lower
Little River [segment revised for renewal]. The Lower Little River revised segment 11-69-(5.5) is not
listed as "impaired" [not included on North Carolina's 2014 Final 303(d) list]. However the segment
immediately upstream [11-69-(0.5)] lists the fish community only "Fair" [Nar, AL, FW], see attached.
Compliance Inspection. Mooresville Regional Office (MRO) inspected the facility on
December 5, 2013.
COMPLIANCE REVIEW
Whole Effluent Toxicity (WET) Testing. The Town of Taylorsville WWTP conducts Quarterly
WET testing [Chronic; Ceriodaphnia 7-day pass/fail @ 8.2%). A review of toxicity data, submitted
from the years 2010 through 2014 indicated all twenty (20) WET tests reported as "pass."
Notices of Violation (NOVs) and Penalty Assessment. BIMS indicates this facility received no
effluent limit violations; no NOVs or civil penalties were assessed during the last permit cycle.
