On September 15, 1989, the Division of Environmental Management issued NPDES Permit
No. NCO075141 to Mr. Hal L. Bond. A review of the permit file has indicated that an error was
inadvertently made in the permit. Accordingly, we are forwarding herewith modifications to the
subject permit to correct the error. Those _permit-mdifichtions are_ issued-,fo dele.te_,pHI

Please find enclosed an amended effluent limitations page which should be inserted into your
permit. The old effluent limitations page should be discarded. All other terms and conditions
contained in the original permit remain unchanged and in full effect. These permit modifications
are issued pursuant to the requirements of North Carolina General Statutes 143-215.1 and the
Memorandum of Agreement between North Carolina and the U. S. Environmental Protection
Agency.
If any parts, measurement frequencies or sampling
modification are unacceptable to you, you may request
Regulation 15 NCAC 213.0508(b) by written request to
issues to be contended. Unless such request is made
permit, this permit shall be final and binding. Should you
right to request an adjudicatory hearing.

The chlorinator shall be inspected weekly to ensure there is an ample supply of chlorine tablets for continuous disinfection of the effluent.
The pH shall not be less that 6.0 standard units nor greater than 9.0 standard units.

Our records indicate that NPDES Permit No. NC 0075141 was
issued on September 15, 1989 for the discharge of wastewater to
the surface waters of the State from your facility. The purpose
of this letter is to advise you of the importance of the Permit
and the liabilities in the event of failure to comply with the
terms and conditions of the Permit. If you have not already done
so, it is suggested that you thoroughly read the Permit. Of
particular importance is Page M3.
Page M3 sets forth the effluent limitations and monitoring
requirements for your discharge(s). Your discharge(s) must not
exceed any of the limitations set forth. The section headed
"Monitoring Requirements" describes the measurement frequencies,
sample types and sampling locations. Upon commencement of your
discharge (or operation), you must initiate the required
monitoring. The monitoring results must be entered on the
reporting forms furnished to you by this Agency. If you have not
received these forms, they should be arriving shortly. If you
fail to receive the form, please contact this Office as quickly
as possible. I have enclosed a sample of the "Effluent"
reporting form (DEM Form MR-1), plus instructions for completing
the form. It is imperative that all applicable parts be
completed, and the original and one copy be submitted as
required.
The remaining Parts of the Permit (Parts II and III) set
forth definitions, general conditions and special conditions
applicable to the operation of wastewater treatment facilities
and/or discharge(s). The conditions include special reporting
requirements in the event of noncompliance, bypasses, treatment

unit/process failures, etc. Also addressed are requirements for
a certified wastewater treatment plant operator if you are
operating wastewater treatment facilities. Any changes in
operation of wastewater treatment facilities, quantity and type
of wastewater being treated or discharged, expansions and/or
upgrading of wastewater treatment facilities must.be permitted or
approved by this Agency.
Failure to comply with the terms and conditions of an NPDES
Permit subjects the Permittee to enforcement action pursuant to
Section 143-215.6 of the North Carolina General Statutes. A
civil penalty of up to $10,000 per violation plus criminal
penalties may be assessed for such violations. If you find at
any time that you are unable to comply with the terms and
conditions of the Permit, you should contact this Office
immediately. A Consent Order may be necessary while pursuing
action to obtain compliance.
As a final note, an NPDES Permit is normally issued for a
five-year period. Permits are not automatically renewed.
Renewal requests must be submitted to this Agency no later than
180 days prior to expiration. Please make note of the expiration
date of your Permit. This date is set forth or. Page M•-1 .or T-•1
of the Permit. Also note that NPDES Permits are not
transferable. If you, as the Permittee, cease to need this
Permit, then you should request that the Permit be rescinded.
As mentioned previously, the purpose of this letter is to
advise you of the importance of your NPDES Permit. Please read
the Permit and contact this Office at 704/663-1699 in Mooresville
if you have any questions or need clarification. We look forward
to providing any assistance.

1. INTRODUCTION
The following is an Engineering Proposal of treating domestic
wastewater from a residence owned by Hal L. Bond located in
Alexander County, North Carolina. The residential lot is located
in the Taylorsville Beach Community of the County. The Catawba
River is dammed at this area forming the easting reach of Lake
Hickory .
2. DESCRIPTION
The wastewater from the residential site will be strictly domestic
in form and will be generally the same characteristics as that of
other residential units throughout the area. The residential site
will be used to construct a three (3) bedroom unit. Using the
flow of one -hundred and fifty gallons per day (150 gpd) for each
bedroom it would be expected that the. average daily flow would be
four -hundred fifty gallons per day (450 gpd).
3. WASTE TREATMENT AND DISPOSAL OPTIONS
In February, 1988, the site was examined by the Alexander County
Sanitarian and the soil was found unsuitable for a ground _
absorption system. Reasons for this finding are as follows:
a. The western and central portions of the lot are
unsuitable due to slope greater than 30 percent;
lack of provisionally suitable or suitable soil
depth; unsuitable placement and texture of material
in filled areas; and required separation distances
from the well, and property lines. (paragraphs
.1940[c[; .1941[4] [a] [ii] and .1943; .1950[b]; and
.1950[a]).
b. The eastern and southern portions of the lot is
unsuitable due to the required separation distance
from any stream and lake or impoundment (paragraph
.1950[a]).
C. An area in the central portion of the lot did appear
to be undisturbed with respect to extensive grading,
however, this area is unsuitable due to soil depth,
(paragraph .1943).
In July, 1988,'the site was evaluated for and found unsuitable fur
using the Pre -fabricated, Permeable Block Panel System.
A final option is to flow the wastewater through a septic tank, a
graded sand filter media system, a calibrated chlorine system and
then discharge into Lake Hickory at the property line.

This system will be reasonable cost effective since gravity can be
used to flow the wastewater through the treatment system to the
lake. Also, other treatment systems would be mechanical or
chemical which would be 'unreasonable for a single family
residential unit.
4. DETAILED DESCRIPTION
The' wastewater from the residential unit will flow through a 6"
PVC sanitary sewer line to a properly sized septic tank. The
septic tank will be complete with sanitary tees, baffle,
inspection ports and outlet.' At an appropriate hydraulic,
elevation below the septic tank, an appropriately sized sand
filter will be constructed. The filter will have properly sized
media, inflow piping system on top and outflow piping system
below. The filter will be protected by an impermeable membrane as
may be required. Again, at the appropriate elevation down stream
a tablet -type chlorinator will be installed in -line. This will be
.located'and sized to provide the appropriate chlorination. The .
final line section will flow the treated wastewater into>the lake
bed at an appropriate location from the normal pond level both
,horizontally and vertically. Standard detailed plans and
specifications will be provided for permit approval and
construction.
