R. 1135 in southern Alexander County, travel northwest on S. R. 1135 approximately 0.8 mile. Turn left onto S. R. 1136 (Taylorsville Beach Road) and travel approximately 0.37 mile. The vacant site is located on the left side of S. R. 1136. 6. Discharge Point - Latitude: 350 49' 37" Longitude: 810 12' 13" Attach a USGS Map Extract and indicate treatment plant site and discharge point on map. USGS Quad No.: D 14 SW 7. Size (land available for expansion and upgrading): There is ample area available for construction of the proposed facilities. 8. Topography (relationship to flood plain included): Hilly, 3-12o slopes. Proposed site is above the lake's high water elevation. 9. Location of Nearest Dwelling: Several within 300 feet of the site. 10. Receiving Stream or Affected Surface Waters: Unnamed tributary to Catawba River (Lake Hickory) a. Classification: WS-III, B b. River Basin and Subbasin No.: Catawba 030832 I Page Two C. Describe receiving stream features and pertinent downstream uses: Discharge will enter a small cove of Lake Hickory used frequently for a variety of recreational purposes such as swimming, boating, fishing, etc. This area experiences frequent human contact as many private residences are located on either side of the cove. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS, 1. Type of wastewater: 100o Domestic 0% Industrial a. Volume of Wastewater: 0.00036 MGD* *based on three bedroom home b. Type of Treatment (specify whether proposed or existing): The proposed facilities will consist of a 1000 gall -on capacity septic tank followed by a subsurface sand filter and tablet disinfection: K 5. Special monitoring requests: None at this time 3. Additional effluent limits requests: None at this time. 4. Other: N/A PART IV —EVALUATION AND RECOMMENDATIONS Mr. Hal L. Bond proposes to construct a three (3) bedroom residence on a vacant lot located at the back of a small cove on Lake Hickory. The County Health Department has certified that Page Three no non -discharge options are available at this site. A septic tank/sand filter has been proposed with the applicant having the option of discharging to a small unnamed tributary or directly into Lake.Hickory. The writer evaluated both potential discharge locations and the possible effect on surface waters. Discharge location No. 1 appears to be the most favorable location, however, to discharge at this point the applicant must extend his outfall line approximately 150 feet out into the cove. This point should provide adequate mixing and assimilation of the effluent which cannot be achieved at the back of the cove. Discharge location No. 2 was also evaluated; however, 7Q10 and 30Q2 estimates on the receiving stream are 0.0 cfs and based on current Division policy, denial is recommended at this location. The proposed facilities should be sufficient to comply with the recommended effluent limitations (see attached Waste Load Allocation). The proposed sand filter bed should be lined to prohibit groundwater contamination and an effluent sampling port included in the design prior to issuance of an Authorization to Construct. A subsurface sand filter is not considered acceptable. Pending final approval by Permits and Engineering and Technical Services, it is recommended that an NPDES Permit be issued for discharge location No. 1.
