Wes Bell last inspected the facility on March 21, 2001.
Permit:
The permit for this facility expired on June 30, 2000. A permit renewal application was
submitted by the permittee as required by 15A NCAC 2H .0105(e) and .0106(b); however, a new
permit has not been issued. The facility continues to discharge under the conditions of the
expired permit (see Part II, Section B, 10 of the NPDES Permit).
The permitted facility designated as outfall 001 is a 0.780 MGD wastewater treatment
plant (WWTP) consisting of a bar screen, instrumented flow measurement, aeration basin with
two mechanical aerators, a clarifier, sludge return pumps,, aerobic digester, cascade post aeration,
and sludge drying beds. Outfa11002 consists of two settling ponds connected in series for water
plant filter backwash.
The permit description of the WWTP (outfall 001) should be amended to include
polymer, phosphoric acid, and ammonium sulfate addition facilities.
Facility Site Review/Operation and Maintenance:
The Class H facility is staffed 8 hours per day, 5 days per week by the ORC who holds a
Grade IV WWTP Operator's Certification. The backup operator also has a Grade IV WWTP
Operator's Certificate.
The facility appeared to be well operated and maintained. A comprehensive process
control program consisting of dissolved oxygen, pH, MLSS, MLVSS, sludge settleability, and
sludge centrifuge, and sludge blanket depth measurements is used in the operation of the facility.
Bar screenings are combined with the dried sludge and taken to the Foothills Landfill in
Lenoir,'North Carolina.
Laboratory:
Water Tech Laboratories, Inc. (Certification # 50) is contracted to conduct the required
testing and analyses. The on -site laboratory was granted a certification exemption by, the
Division's Laboratory Certification Unit for pH, TRC, and settleable solids on October 3; 1996.
Be advised that laboratory certification for field parameters is required accordingto temporary
rules [15A NCAC 2H .0804(g)] adopted by the Environmental Management Commission that
became effective on October 1, 2001.
Records/Reports:
Records and reports consist of ORC/operations and maintenance log, process control
data, monthly monitoring reports, chain of custody forms, laboratory reports, sludge records,
flow charts, and instrument calibration logs. All records/reports were complete and up-to-date.

Flow Measurement:
The influent flow is measured continuously by means of an ISCO 3230 bubble tube flow
meter. The flow meter is calibrated twice yearly by ISI Instrumentation. The last calibration
was conducted on January 9, 2002.
Self -Monitoring Program:
Monthly self -monitoring reports were reviewed for the period March 2001 through
February 2002. No violations were reported during the period.
Composite samples are collected by time -based methodology. The Division has granted
an exemption from the flow proportional composite -sampling requirement provided that the
discharge flow does not vary by more than 30% (See the permit modification cover letter dated
May 26, 1998).
Sludge Disposal:
Republic Services, Inc. takes dried sludge to the Foothills Landfill in Lenoir, North
Carolina for disposal.
Effluent/Receiving Waters:
The effluent discharge (outfall 001) was clear with a trace amount of suspended solids
and foam. The foam dissipated completely within 100 feet of the discharge. The receiving
stream, Muddy Creek (CTB32, 7Q10 =1.4 CFS), did not appear to be impacted by the
discharge. A 24-hour composite sample was.collected of the discharge for use in a chronic
Ceriodaphnia dubia pass/fail toxicity test. Toxicity testsesults will be forwarded under separate
cover when available.
The WTP discharge (outfall 002) was clear and did not appear to have any impact on the
receiving stream.
