Dear Mr. Bollinger:
Enclosed is a copy of the Compliance Evaluation Inspection (CEI) for the
inspection conducted at the subject facility on June 17, 2008, by Ms. Barbara Sifford with
this Office. Thank you for your assistance and cooperation during the inspection. The
system at the time of the inspection did not have a discharge therefore no analytical had
been performed. The chlorinator and discharge box were dry and there is no presence of
surface discharge. Should this system ever discharge samples should be collected and.
analyzed. If the system fails and causes any surface water impacts or discharges to the
ground surface the system will have to be repaired to original design specifications.
The enclosed reports should be self-explanatory.

No analytical has been performed since no flow is appartent from the system. Inspection from this office in
1992 indicated no flow at that time. Meter readings from the City of Hickory watermeter indicate about
3000 gallons per month are used at the residence. The wastewater has to be leaking underground either
from the tank or the sand filter. No discharge to the lake or to the surface of the ground is apparent. No
environmental impacts can be detected, should this ever occur the system will have to be repaired to design
specifications.

3000 gallons. Septic tank has been pumped several months ago.
