Any violation of these terms may subject to fines, imprisonment or civil action; or may cause the permit to be null and void. 1) Unless specifically altered herein, this minor modification authorizes A) the expansion of the concrete boat launch pad, B) the reconfiguration and extension of the docking facility, all as expressly and specifically set forth in the attached workplan drawings, and C) to add 140 square feet to three previously authorized finger piers. 2) This minor modification shall be attached to the original Permit No. 131-08, which was issued on 8/25/08 and copies of both documents shall be readily available on site when Division personnel inspect the project for compliance. 3) All conditions and stipulations of the active permit remain in force under this minor modification unless specifically altered herein. (See attached sheet for Additional Conditions) This permit action may be appealed by the permittee or Signed by the authority of the Secretary of DENR and the other qualified persons within twenty (20) days of the issuing Chairman of the Coastal Resources Commission. date. An appeal requires resolution prior to work initiation or continuance as the case may be. This permit must be accessible on-site to Department personnel when the project is inspected for compliance. Any maintenance work or project modification not covered hereunder requires further Division approval. V Ja H. A permit modification is requested for the following: • Reconfigure permitted slips • Increase size of concrete boat launch pad from 5218 sf to 8574 sf The drawings have been revised to reflect the changes and are attached. Sheet 06 is the permitted slip configuration and Sheet 06a shows the proposed slip reconfiguration and expanded launch pad. The total percent impervious area in the AEC is below the DCM 30% threshold. Included with this letter is a check for $100.00. The Stormwater plan has been modified from the original permit and has been submitted to DWQ for approval. Discharge from the Stormwater treatment devices will be piped through the bulkhead at the locations shown on the plans. The easternmost outfall pipe will discharge into the water. The westernmost outfall pipe will be discharged behind the wetlands and through a level spreader before entering the wetlands. The level spreader is designed to reduce flow to a velocity less than 2 fps. Sheet 11 shows a section of the level spreader. Your attention to this matter at your earliest convenience is appreciated. If it becomes necessary or advisable, we would be glad to meet the DCM staff to discuss this modification. Please call me if you have any questions or need additional information.
