Authorization to Construct
Willow Oak Area Mobile Home Park
Rockingham County
Dear Mr. Vogler:
A letter of request for Authorization to Construct was received September
12, 1984, by the Division and final plans and specifications for the subject
project have been reviewed and found to be satisfactory. Authorization is hereby
granted for the construction of a 0.0175 MGD wastewater treatment facility consisting
of an inlet bar screen, a two cell wastewater stabilization pond with a total
____volume_of-_1..81_ MG, _chlorination. and associated appurtenances._
This Authorization to Construct is issued in accordance with Part III
paragraph C of NPDES Permit No. NCO060259.issued November 1, 1984, and shall
be subject to revocation unless the wastewater treatment facilities are constructed
in accordance with the conditions and limitations specified in Permit No. NCO060259.
The Permittee must employ a certified wastewater operator in accordance
with Part III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of
in accordance with General Statutes 143-215.1 and in a manner approvable by the
North Carolina Division of Environmental Management.
.In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the applicant shall take such immediate corrective action
as may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
The lagoon should have a 12-inch liner with a minimum permeability of 10-6
cm/sec when capacted to 95% Proctor dry density. The consultants report suggests
using one of the three on -site soils (CL, MH-CH, CH) be used as liner material.
Since the presence of sand is undesirable in liner construction, the selection
of liner material should be limited to CH or, less perferably, MH-CH soils.

Two (2) monitor wells (1 upgrade, 1 downgrade) .should be installed. The wells
should follow the details of the attached diagram and should screen the interval
from 15 to 20 feet below.land surface. The location of wells should be by approval
of the Winston-Salem Regional Office.
The wells should be sampled, prior to construction and every March, July and
November thereafter, for the following parameters: NO3, Total Ammonia, TDS, TOC,
Cl, pH, Conductance, Water Levels (this measurement should be made immediately
prior to any pumping/bailing for groundwater quality samples).
The applicant must comply with Special Order by Consent No. 85-01, which includes
a timetable for meeting the effluent limits of 1 MG per liter for phosphorus. All
final plans and specifications for phosphorus removal facilities must be submitted
to the North Carolina Division of Environmental Management by March 31, 1987.
