Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended). The monitoring -frequency has been changed from three times per week to weekly to reflect the monitoring frequency of a Class II facility. This permit includes a limit for Enterococci that will take effect on March 1. 2008. Additionally, this permit includes a TRC limit that will take effect on March 1. 2009. If you wish to install dechlorination equipment, the Division has promulgated a simplified approval process for such projects. Guidance for approval of dechlorination projects may be viewed online at http://w .nccgl.net/news/ATCoverview.html. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding. Please note that this permit is not transferable except after notice to the Division. The Division may require modification or revocation and reissuance of the permit. Enclosed is the application and the sludge management plans for the facility. There have been no additions to the plant since the last permit renewal. I would like to request that you modify my permit, to change the daily maximum on the BOD from 7.5 mg/1 to 10.0 mg/1. On a few occasions we have exceeded the 7.5 mg/l for some unknown reason. As you can see from our annual daily maximum average is only 5.3 mg/l. I would also like the permit to be modified, reducing the monitoring frequency from three times a week to once a week. This plant is only 40,000 gallons per day and has had only a few minor violations over the years. Thank you for your consideration. Section 1001 provides a punishment by a fine of not more than $25,000 or imprisonment not more than 5 years, or both, for a similar offense.) 3 of 3 Form-D 4105 Sludge Management Plan When our sludge settles in a jar for about 20 minutes at 40 — 45%, we start removing it by the clarifiers return. It is pumped to the sludge holding tank to be aerated and thickened. When the tank is full, the air is cut off for about six hours. The clear water on top is then pumped back to the head of the plant. The air is cut back on and the tank is refilled. This process is continued until there is not enough water to pump off. Hydrated lime is then mixed with the sludge until a pH of 12 is maintained for at least 2 hours. The sludge is remove to permitted application fields and applied. % Solids, TCLIPs and soil testing is done at least annually. Cover crops are grown and harvested on these fields.
