Ted,
I apologize that the application was missing some items. We had to have the application reviewed by the City
and County prior to submitting to you so I thought we had it filled out appropriately. I have corrected these
items and attached a revised pdf.
You are also in possession of the permit for the public portion of the proposed sewer as well (WQ0041967 Ver
A), I corrected these items in that application as well and have attached a pdf. Thank you for helping us with
this and please let me know if I can provide any additional information to assist.
Thanks,

Subject: RE: [External] FW: 55 at The Park - Private fast track sewer extension application
We are reviewing the subject application and there are several items on the application that need completion before we
can continue.
The application was signed by the applicant and certified by the professional engineer that it was accurate and
complete. However, multiple sections were not completed. In the future, please make sure to review the application
for accuracy/completeness before certifying and submitting it.
Item 11.4 did not contain the latitude and longitude of the project as required.
Item IV was not completed as required.
Item V was not completed as required.
Item IX.1 was not completed as required.
A Developer's Operational Agreement was included for this project. The vast majority of projects we see do not have
these agreements; we just want to confirm that a HOA will be formed and will assume ownership/liability of the sewer
line as specified in the Developer's Operational Agreement. The developer shall not transfer ownership or relinquish its
responsibility for the operation and maintenance of the sewer line until the FINAL engineering certification has been
submitted and a permit has been reissued to the Developers successor.
We will continue review of this application and drafting of the permit upon submittal and review of the above.

Ted, I hope you are doing well. Thank you for helping us to keep this application moving. 1 wanted to check to see
where we are in your review process, so that we can update our construction schedule.
I hate to bug you but as you may know we lost some staff in the spring which delayed our submittal, putting this part of
the project on hold. Once we get your approval and the City completes their paperwork, we can get the contractor
started, reducing further delays.
Thanks again for your help,

Ted, I am sorry for the confusion, I assume that the application was filled out with PulteGroup listed as
the applicant. That name is our National Corporate brand name, however in North Carolina we operate
as "Pulte Home Company, LLC'. That is the name that should have been listed on the application.
Attached is the Corporate Resolution denoting Chris Raughley as a person with signing powers on behalf
of Pulte Home Company, LLC.
Please let us know if we should have Chris, Matt or myself come to your office to correct this
information on the application.

I apologize that the application was missing some items. We had to have the application reviewed by the City
and County prior to submitting to you so I thought we had it filled out appropriately. I have corrected these
items and attached a revised pdf.
You are also in possession of the permit for the public portion of the proposed sewer as well (WO0041967 Ver
A). I corrected these items in that application as well and have attached a pdf. Thank you for helping us with
this and please let me know if I can provide any additional information to assist.

Ted,1 hope you are doing well. Thank you for helping us to keep this application moving. I wanted to check to see
where we are in your review process, so that we can update our construction schedule.
I hate to bug you but as you may know we lost some staff in the spring which delayed our submittal, putting this part of
the project on hold. Once we get your approval and the City completes their paperwork, we can get the contractor
started, reducing further delays.
Thanks again for your help,

Based on the current guidance to minimize the spread of COVID-19, DEQ has adjusted operations to protect the
health and safety of the staff and public. Many employees are working remotely or are on staggered shifts. To
accomodate these staffing changes, all DEQ office locations are limiting public access to appointments only. Please
check with the appropriate staff before visiting our offices, as we may be able to handle your requests by phone or
email. We appreciate your patience as we continue to serve the public during this challenging time.

Ted, I am sorry for the confusion, I assume that the application was filled out with PulteGroup listed as
the applicant. That name is our National Corporate brand name, however in North Carolina we operate
as "Pulte Home Company, LLC. That is the name that should have been listed on the application.
Attached is the Corporate Resolution denoting Chris Raughley as a person with signing powers on behalf
of Pulte Home Company, LLC.
Please let us know if we should have Chris, Matt or myself come to your office to correct this
information on the application.
Thankyou
Randy

Matt and Chris,
We received the subject application 9-11-20. A review of the NC Secretary of State's website does not
show an entity named "Pulte Group". Please provide the proper legal name for the applicant for the
project, and ensure the applicant's signature authority name (Chris Raughley, Developer) complies with
15A NCAC 02T .0106(b). (see below)
