This office has reviewed the subject erosion and sedimentation control plan. We find the plan to be
acceptable and hereby issue this Letter of Approval with Modifications. The enclosed Certificate of
Approval must be posted at the job site. This plan approval shall expire three (3) years following the
date of approval, if no land -disturbing activity has been undertaken, as is required by Title 15A NCAC
40.0129.
As of April 1, 2019, all new construction activities are required to complete and submit an electronic
Notice of Intent (N01) form requesting a Certificate of Coverage (COC) under the NCG010000
Construction Stormwater General Permit. This form MUST be submitted prior to the commencement
of any land disturbing activity on the above named project. The N01 form may be accessed at
deq.nc.gov/NCG01. Please direct questions about the N01 form to Annette Lucas at
Annette.ltica5@ncdenr.gov or Paul Clark at Paul.clarlt a ncdgnr.gay. After you submit complete and
correct NOI form, a 'CPOC will be emailed to you within three busyness days. Initially, DEMLR will
not charge a fee for the coverage under the NCG01 permit. However, on or after June 1, 2019, a $100
fee will be charged annually. Tkis fee is to be sent to the DEMLR Stormwater Central Office staff in
Raleigh.

Title 1SA NCAC 413.0118(a) and the NCG01 permit require that the following documentation be kept
on file at the job site:
1. The approved E&SC plan as well as any approved deviation,
2. The NCG01 permit and the COC, once it is received.
3. Records of inspection made during the previous 30 days.
Also, this letter gives the notice required by C.S.113A-61.1(a) of our right of periodic inspection to
insure compliance with the approved plan.
North Carolina's Sedimentation Pollution Control Act is performance -oriented, requiring protection of
existing natural resources and adjoining properties. If, following the commencement of this project, the
erosion and sedimentation control plan is inadequate to meet the requirements of the Sedimentation
Pollution Control Act of 1973 (North Carolina General Statute 113A-5I through 66), this office may require
revisions to the plan and implementation of the revisions to insure compliance with the Act.
Acceptance and approval of this plan is conditioned upon your compliance with Federal and State water
quality laws, regulations, and rules. In addition, local city or county ordinances or rules may also apply to
this land -disturbing activity. This approval does not supersede any other permit or approval.
Please note that this approval is based in part on the accuracy of the information provided in the
Financial Responsibility Form, which you provided. You are requested to file an amended form if
there is any change in the information included on the form. In addition, it would be helpful if you
notify this office of the proposed starting date for this project. Please notify us ifyou plan to have a
preconstruction conference.

1. Provide copies of accessfencroachment agreements from NCDOT and other public or
private party entities. (GS 113A-54.1(a))
2. Provide a detail e&speci fi c construction sequence related to erosion and sediment control
(including installation of critical measures prior to the initiation of the land -disturbing
activity, removal of measures after areas they serve are permanently stabilized, and the
conversion of temporary structures to permanent structures). Provide a specific sequence
for the removal of the basin (including the use of a silt bag for dewatering). Address the
installation and removal of inlet and outlet protection. Remove sequence #14 as it
conflicts with the NPDES groundcover requirements. (15A NCAC 04B. 0106(5))
3. Provide a legend with symbols.'icons to identify all proposed erosion and sedimentation
control measures. Measures are to be to scale. The Iine type for the limits of disturbance
and the perimeter silt fence are to be separate and easily distinguishable. The symbol in
the legend for the concrete washout is mislabeled as riprap slope protection. The symbol
provided in the legend for the basin is not the symbol used on the plan sheet for the

4. Relocate the stockpiletstaging area to be 50 feet away from the temporary diversions.
Provide a break in the silt fence (on the upgrade side) for access. (GS 113A-57(3), 15A
NCAC 04B .0105, 15A NCAC 04B .0106(5), GS 113A-57(4), NCGO I Section 1113 i (d))
5. Provide additional erosion and sedimentation control measures as required to protect all
public and private property from damage. Concentrated flow must be diffused to prevent
erosion and offsite sedimentation. Provide perimeter silt fence along downgrade areas (i.e.
around the basin). Provide silt fence outlets at low points (i.e. beneath the skimmer outlet
pipe). (GS 113A-57(3), 15A NCAC 04B .0105)
6. Provide stable conveyance (extent pipe, provide tarp or other acceptable practice)
from outlet of skimmer to perimeter silt fence/silt fence outlet. (15A NCAC
0413.0106(4))
7. Provide two copies of the plan set (one full-size and one half-size), showing the
modifications required for approval prier to initiating land disturbing activity. Provide
a cover letter referencing the project name, project ID, and purpose for submittal. (G.S.
113A-57(4))
