Dear Ms. Karoly,
We received notification in a letter dated 20 March 2007 that the processing of the permit
application for the above referenced property has been placed on hold pending submittal of
additional information. The attached information is provided as requested.
Incorrect andlor missing wetland, intermittent perennial stream andlor buffer impacts.
Section VI (1) of the PCN that was submitted has been revised to give the specific impact
area of each impact (see attached). While collecting this information, it was noted that there is a
fourth impact area that was not previously highlighted on the map, though the total amount of
impacts associated with the sewer line remains 11.832 SF (.2716 AC). Impact Area 1 is 2,650
SF (.0608 AC), impact area 2 is 72 SF (.0017 AC), impact area 3 is 7,800 SF (,1790 AC), and
impact area 4 is 1,310 SF (.0301 AC). A revised map showing each impact area is also attached.

impacts. Stream impacts include, but are not limited to placement of fill or culverts, dam
construction, flooding, relocation, stabilization activities (e.g., cement walls, rip-rap, crib
walls, gabions, etc.), excavation, ditching/straightening, etc. If stream relocation is proposed,
plans and profiles showing the linear footprint for both the original and relocated streams
