However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on June 19, 2020. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy. Application (15A NCAC 02T .0105): 1. Item II.4. indicates the facility is a privately -owned public utility. If yes, provide a copy of the Certificate of Public Convenience from the North Carolina Utilities Commission. 2. Item III.1. requires an up-to-date site map. This site map is necessary to verify that all setbacks are being met and display the location of monitoring wells. The attached map is missing many of the requirements. Please provide a map that shows all wells, surface waters (including wetlands and ditches), wetted perimeter of all disposal fields, property lines, and compliance/review boundary. 3. Table IV. 1. requires well coordinate accuracy to 6 decimal degrees. The gradient and location in relation to compliance/review boundary is also required. B. Groundwater 1. Per the Regional Office staff report, monitoring wells MW-3 and MW-7 have recurrent ammonia exceedances and MW-6 has some exceedances of ammonia. Address these groundwater exceedances including the following: a. Evaluating the current groundwater monitoring plan. b. Installation of new monitoring well(s), as requested by the Regional Office. c. Wells should be located on the review boundary, unless exceedances have been found, in which case, wells should be located on the compliance boundary. d. Any earthen storage structures without a liner must have a compliance/review boundary.
