This submittal package includes one original and one copy of all required documents. An electronic copy of this application was uploaded via the Non -Discharge Branch website. This permit is scheduled to expire on April 30, 2021. The current permit, and past revisions, have had no requirement to install monitoring wells, thus monitoring well data is not applicable. Due to the eroded condition of the land application area and that of the marsh area surrounding it, installing new monitoring wells would cause further damage to the site that is already considered in a condition of non-compliance. Over the past 17 years, this site has operated, on average, at 25% the daily effluent flow of 1,750 gallons per day. It is for these two reasons that we ask that monitoring wells not be a requirement of this renewal. For additional information, please do not hesitate to contact Mr. Timothy Lawrence of the Environmental Affairs Department at (252)466- 2754.
