This facility is in violation of its' permit conditions for operation and maintenance. 3. There were several roof drains that were directed to the forebay as well as the storage pond. All flow going into the system should enter by way of the inlet structures to the forebay. Flows that enter elsewhere in the system will allow the flow to short circuit and not receive the treatment that it was designed to due to decreased detention times. 4. There were areas of erosion that should be repaired as required by your permit. 5.' The orifice in the outlet structure appeared to be plugged with lawn debris, and trash. You are required to visit the site after every significant runoff producing rainfall event and at least monthly for sediment accumulation, erosion, trash accumulations, vegetated cover and general condition. 6. There appeared to be an excessive amount of sediment in the forebay. You should check the elevations to determine if sediment must be removed. Please refer to your signed Operation and Maintenance agreement for your requirements. 7. Operation and Maintenance records were not available on the inspection date. If you have such records, please submit the last three months of data along with your response to this notice. Be advised that your permit requires that records be kept. 8. The sideslopes and rip -rap are placed where previously approved by Wilmington Regional Office Staff. The report summarizes the findings of a recent inspection of the project's stormwater controls conducted on November 21, 2003 to determine compliance with Stormwater Permit Number SW8 970616 issued on may 20, 1997. A summary of the findings and comments are noted under Section 4 of the report. Please inform this Office in writing before December 21, 2003, of the specific actions that will be undertaken and the time frame required to correct the deficiencies. This area must be stabilized with grass or rip/rap. (b) Both Detention ponds are not being maintained as specified in the permit.
