Permeable Pavement Operation and Maintenance Agreement
I will keep a maintenance record on this BMP. This maintenance record will be kept in a
log in a known set location. Any deficient BMP elements noted in the inspection will be
corrected, repaired or replaced immediately. These deficiencies can affect the integrity
of structures, safety of the public, and the removal efficiency of the BMP.
At all times, the pavement shall be kept free of:
— Debris and particulate matter through frequent blowing that removes such
debris, particularly during the fall and spring.
— Piles of soil, sand, mulch, building materials or other materials that could deposit
particulates on the pavement.
— Piles of snow and ice.
— Chemicals of all kinds, including deicers.
The permeable pavement will be inspected once a quarter. Records of operation and
maintenance will be kept in a known set location and will be available upon request.

be repaired immediately.

Required Attachments:
1. Legible vicinity map.

3. PDF formatted site plan with the boring locations to be tested. Site plans should
be emailed or hand -delivered only. Illegible faxed maps will not be accepted.
All proposed infiltration areas and existing, active utility lines located within the proposed
basin/trench must be marked and flagged. If these areas are not flagged, the Soils Scientist
reserves the right to decline to do the investigation. If the proposed infiltration system will be
located in an area of existing pavement and there is no open area nearby, equipment capable
of breaking through the impervious layer must be provided. The soils investigation does not take
the place of a soils report prepared by an appropriate professional. The Soils Scientist will only
verify the soil conditions that are reported in the Soils Report, and make a determination as to
the suitability of the site to meet the infiltration design requirements under NCAC 2H.1000, and
assumes no liability should the system fail.

On March 23. 2011 1 made an evaluation of the proposed storm -water infiltration at the .
above site. T'he purpose of his evaluation was to describe the soil profile, qualify the
depth to the seasonal high water table to determine soil texture, to identify the infiltration
rate and to inform the project engineer of any potential limitations to this project.
The soil on this site is mapped Baymeade in the Soil Survey of Brunswick County. North
Carolina Soil Conservation Service Book dated 1986. Actual soil borings of Hole 91
confirm -the soil to be in the Baymeade Soil Series. Hole 42 appears to be in the
Bavmeade/Nlandarin So-il Series.
The Baymeade/Mandarin soil on this site can be described as well -drained to moderately -
well soil on low ridges and convex divides. Most areas are generally broad and long.
The seasonal high water table is normally evident by observation of redoximorphic
features and soil color suggesting past condition of saturation and reductiot). 1'bere is
evidence of soil color from the two holes that I dug and it is my opinion that the seasonal
high water on Hole # 1 is 32 inches, Hole #2 is 28 inches from the natural soil surface (see
map for location of holes).
It is expected from the soil borings that the infiltration rate of Hole 91 is 6.0 to 20.0
inches hour from 0 to 32 inches and 2.0 to 6.0 inches/hour from 32 to 60 inches. Hole #2
is'6,0 to 20 inches/hour fronn 0 to 28 inches and 1.0 to 2 inches/hour from 28 to 60 inches
i'rom the natural soil surface.
In summary, these test holes were placed on this parcel upon engineer's request. There is
only a minor difference in elevations throughout this parcel (see attachment #2). The
engineer is designing a building for the Calabash Fire Department and this information
was provided to assist the engincer-in designing the storm -water for this project.
