In accordance with the provisions of Article 21 of Chapter 143, General Statutes of
North Carolina as amended, and other applicable Laws, Rules and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction, operation and maintenance of an overall 30% low density project with a
curb outlet system and development of high density pockets with treatment by four
hydrodynamic separators, each followed by a 100' long minimum grassed swale, in
compliance with the provisions of 15A NCAC 2H .1000 (hereafter referred to as the
"stormwater rules' and the approved stormwater management plans and
specifications, an other supporting data as attached and on file with and approved by
the Division of Water Quality and considered a part of this permit.
Please note in particular the operation and maintenance requirements of Section 11.4 of
this permit.
The Permit shall be effective from the date of issuance until June 27, 2016 and shall be
subject to the following specific conditions and limitations:
1. DESIGN STANDARDS
1. This permit is effective only with respect to the nature and volume of stormwater
described in the application and other supporting data.
2. This permit covers the construction of 378,181 square feet of built -upon area as
follows: (a) OS buildings- 104,308 sq ft; (b) OS parking and driveways- 223,665
sq ft; and (c) OS sidewalks and courtyards- 50,208 sq ft. The hydrodynamic
separator stormwater systems for the four pockets of high density have been
approved for the management of stormwater runoff as described in Section 1.12
on page 3 of this permit. The stormwater control for the four pockets of high
density has been designed to handle the runoff from a total of 125,300 square
feet of impervious area.
3. The overall tract built -upon area percentage for the project must be maintained
at 30%, per the requirements of Section .1005 of the stormwater rules.
4. Approved plans and specifications for projects covered_ by this permit are
incorporated by reference and are enforceable parts of the permit.

5. This project proposes a curb outlet system. Each designated curb outlet swale or
100' vegetated area shown on the approved plan must be maintained at a
minimum of 100' long, maintain 5:1 (H:V) side slopes or flatter, have a
longitudinal slope no steeper than 5 /o, carry the flow from a 10 year storm in a
non -erosive manner, maintain a dense vegetated cover, and be located in either
a dedicated common area or a recorded drainage easement.
6. Runoff conveyances other than the curb outlet system swales and piping for high
density pockets of the tract, such as erimeter ditches, must be vegetated with
side slopes no steeper than 3:1 (H:V�
7. Unless specified elsewhere, permanent seeding requirements for the swales
must follow the guidelines established in the North Carolina Erosion and
Sediment Control Planning and Design Manual.
8. No piping shall be allowed in the low density portions of the tract except those
minimum amounts necessary to direct runoff beneath an impervious surface
such as a road or under driveways to provide access.
9. Roof drains must terminate at least 30' from the Mean high Water line.
10. All stormwater collection and treatment systems must be located in either
dedicated common areas or recorded easements. The final plats for the project
will be recorded showing all such required easements, in accordance with the
approved plans.
11. The runoff from all built -upon area within the permitted, high density pocket
drainage areas of this project must be directed into the permitted stormwater
control system.
12. The following design criteria have been provided in the four VortSentry®
hydrodynamic separators and must be maintained at design conditions.

li. SCHEDULE OF COMPLIANCE
1. Curb outlet swales, vegetated areas and other vegetated conveyances, and the
stormwater management system shall be constructed in their entirety, vegetated,
and be operational for their intended use prior to the construction of any built -
upon surface, per the approved plans.
2. During construction, erosion shall be kept to a minimum and any eroded areas of
the swales or other vegetated conveyances will be repaired immediately.
3. Projects covered by this permit will maintain a minimum 30 foot wide vegetative
buffer between all impervious areas and surface waters.
4. The permittee shall at all times provide the operation and maintenance
necessary to operate the permitted stormwater management systems at
optimum efficiency to include:
a. Semiannual scheduled Inspections {every six months} and as discussed
in following paragraphs.
b. Sediment removal and as discussed in following paragraphs.
C. Mowing, and revegetating of the side slopes.
d. immediate repair of eroded areas.
e. Maintenance of side slopes in accordance with approved plans and
specifications.
f. Cleaning and repair of catch basin grates, flumes, piping, and the flow
spreader mechanism.
A signed Operation and Maintenance Agreement (attached) for both the
curb outlet systems and VortSentry® units are incorporated by reference
and are enforceable parts of this permit.
The VortSentry® units shall be cleaned when the sediment depth has
accumulated to a depth of 3 feet in the treatment sump and as described in
Table 1 of the signed O&M Agreement (attached). The inspection and
maintenance log (page 2 of O&M Agreement) must be completed for each
VortSentry® unit after each rainfall event and maintenance event.
The site and VortSent" units will be subject to inspection by the Division
of Water Quality. If Section 11.4 items a. through f. of this permit are
determined by a Division inspector to be deficient or inadequate, the
Division may impose monitoring requirements for the VortSent" units,
which at a minimum will include influent and effluent monitoring and
reporting of Total Suspended Solids (TSS) for a minimum of 10 rainfall
events over a 12 month period with at least one sampling event in each of
the four quarters, as defined under the Division's Preliminary Evaluation
Period (PEP) for proprietary stormwater treatment technology.
5. The permittee shall submit to the Director and shall have received approval for
revised plans, specifications, and calculations prior to construction, for any
modification to the approved plans, including, but not limited to, those listed
below:
a. Any revision to the approved plans, regardless of size.
b. Project name change.
C. Transfer of ownership.
d. Redesign or addition to the approved amount of built -upon area or to the
drainage area.
e. Further subdivision, acquisition or sale of the project area. The project
area is defined as all property owned by the permittee, for which
Sedimentation and Erosion Control Plan approval was sought.
f. Filling in, piping, or altering any vegetative conveyance shown on the
approved plan.

6. The Director may determine that other revisions to the project should require a
modification to the permit.
7. Records of maintenance activities must be kept and made available to
authorized personnel of DWQ. The records will indicate the date, activity, name
of person performing the work and what actions were taken.
8. The facilities shall be constructed as shown on the approved plans. This permit
shall become voidable unless the facilities are constructed in accordance with
the conditions of this permit, the approved plans and specifications, and other
supporting data.
9. Upon completion of construction, prior to issuance of a Certificate of Occupancy,
and prior to operation of this permitted facility, a certification must be received
from an appropriate designer for the system installed certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications, and other supporting documentation. Any deviations from the
.approved plans and specifications must be noted on the Certification. A
modification may be required for those deviations. The permittee shall submit
the Certification to the Division within 30 days of completion of the project.
10. The permittee is responsible for verifying that the proposed built -upon area does
not exceed the allowable built -upon area.
11. The permittee shall submit all information requested by the Director or his
representative within the time frame specified in the written information request.
12. Filling in or piping of any vegetated conveyances (ditches, swales, etc.)
associated with the permitted project, except for average driveway crossings, is
strictly prohibited by any persons.
13. If the stormwater system was used as an Erosion Control device, it must be
restored to design conditions prior to operation as a stormwater treatment
device, and prior to occupancy of the facility.
14. The Director may notify the permittee when the permitted site does not meet one
or more of the minimum requirements of the permit. Within the time frame
specified in the notice, the permittee shall submit a written time schedule to the
Director for modifying the site to meet minimum requirements. The permittee
shall provide copies of revised plans and certification in writing to the Director
that the changes have been made.
15. The permittee shall submit final site layout and grading plans for any permitted
future areas shown on the approved plans, prior to construction.
16. A copy of the approved plans and specifications shall be maintained on file by
the Permittee for a minimum of 10 years from the date of completion of
construction.
17. The final plats for the project will be recorded showing all required swale
easements and common areas, in accordance with the approved plans.
III. GENERAL CONDITIONS,
Failure to abide by the conditions and limitations contained in this permit may
subject the Permittee to an enforcement action by the Division of Water Qualityy,
in accordance with North Carolina General Statutes 143-215.6A to 143-215.6C.

2. The permit may be modified, revoked and reissued or terminated for cause. The
filing of a request for a permit modification, revocation and reissuance, or
termination does not stay any permit condition.
3. The issuance of this permit does not prohibit the Director from reopening and
modifying the permit, revoking and reissuing the permit, or terminating the permit
as allowed by the laws, rules, and regulations contained in Title 15A of the North
Carolina Administrative Code, Subchapter 2H.1000; and North Carolina General
Statute 143-215.1 et. al.
4. The permit is not transferable to any person or entity except after notice to and
approval by the Director. The Director may require modification or revocation and
reissuance of the permit to change the name and incorporate such other
requirements as may be necessary. A formal permit transfer request must be
submitted to the Division of Water Quality accompanied by the appropriate fee,
documentation from both parties involved, and other supporting materials as may
be appropriate. The approval of this request will be considered on its merits, and
may or may not be approved. The permittee is responsible for compliance with
all the terms and conditions of this permit until the Division approves the permit
transfer.
5. The issuance of this permit does not preclude the Permittee from complying with
any and all statutes, rules, regulations, or ordinances imposed by other
government agencies (local, state and federal) which have jurisdiction. If any of
those permits or approvals result in revisions to the plans, a permit modification
must be submitted.
6. In the event that the facilities fail to perform satisfactorily, including the creation
of nuisance conditions, the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as the construction of
additional or replacement stormwater management systems.
7. Unless specified elsewhere, permanent seeding requirements for the stormwater
control must follow the guidelines established in the North Carolina Erosion and
Control Planning and Design Manual.
8. Approved plans and specifications for this project are incorporated by reference
and are enforceable parts of this permit.
9. The permittee grants permission to DENR Staff to enter the property during
business hours for the purposes of inspecting the stormwater control system and
it's components.
10. The permittee shall notify the Division of Water Quality of any name, ownership
or mailing address changes within 30 days.

This letter is in response to comments in your letter dated May 23, 2006
concerning the above referenced project.
Please find enclosed two (2) copies of Sheet C-1 which has the building
fvlL��e� Jry and parking dimensions on it.
All of the swale and pipe calculations are shown in the_charts_on_S.heet C-
(/ 2. In addition, swa e calculations are shown on the Curb Outlet
-Supplement. Also, please find enclosed the sizing and TSS removal
efficiency information provided by Contech Stormwater Solutions, Inc. for
the four (4) Vortsentry units. We provided the drainage area/flow
information going to the units and they sized the appropriate units and
provide the efficiency information.

4. The underdrain pipe is routed around the stormwater pipes and the
Vortsentry unit and discharges into the rip rap apron at the outlet of pipe
FF3.
You are correct. Once I added the Vortsentry unit, the swale slope
decreased, which also decreased the velocity in the swale. The
information has been revised in the Swale Calculation Chart on Sheet C-
2.

The Wilmington Regional Office received a Stormwater Management Permit Application for
Meadowview Elementary School on May 15, 2006, for review on May 19, 2006. A preliminary
review of that information has determined that the application is not complete. The following
information is needed to continue the Stormwater review:

STORMWATER NARRATIVE ON
MEADOWVIEW ELEMENTARY SCHOOL r�V�

The Meadowview Elementary School is proposed to be located on a 30.4 acre
tract at the corner of Pony Farm Road and Firetower Road in Onslow County. The
runoff from this site flows to an unnamed tributary to Blue Creek, which is classified SC
NSW. The project is proposed to be built under the Low Density Option of the State's
Stormwater Rules. The project built -upon area is 26.8% with an allowable 30% for this
drainage basin.
Stormwater runoff from the paved drives and parking areas will flow through
Curb Outlets and 5:1 grassed swales with a minimum length of 100 feet prior to
intersecting other drainage swales and tieing to existing swales. The roof drainage from
the building will be piped to flow thru 5:1 grassed swales prior to tieing to other swales.
Vortsentry stormwater treatment units have been provided to treat the runoff
from the parking lots prior to discharging to the 5:1 grassed swales.

MEADOWVIEW ELEMENTARY SCHOOL
The Meadowvi::w Elementary School is proposed to be located on a' 30A acre
tract at the corner of i -ony Farm Road and l=iretower Road in Onslow County. The
runoff from this site flaws to an unnamed tributary to Blue Creek, which is classified SC
NSW. The project is proposed to be built under the Low Density Option of the State's
Stormwater Rules. TI e: project built -upon area is 26.8% with an allowable 30% for this
drainage basin.

As we discussed today, Rob and I met with Linda Lewis on 3/24 to discuss the type of
permit for the school's application. It was agreed that a pocket of high density
exists involving the parking areas in the center of the property. Generally, we
consider a high density pocket when built -upon area is approximately 10 percentage
points above the definition of low density or approx 40% BUA for a HD pocket in this
case. You can spread the parking areas to avoid creating a HD pocket, but our
preferred approach is an engineered BMP since school expansions quickly make school
sites high density anyway.
Treatment options for the HD pocket that we discussed with Linda on 3/24 and with you
today include: (a) hydro -dynamic device (we will waive the PEP monitoring
requirements but an O&M agreement will be required); (b) sand filter; (c)
infiltration, and•(d) bioretention.

M. CURB OUTLET SYSTEM MAINTENANCE REQUUREMENTS
1. Mowing will be accomplished as needed according to the season. Grass height will not exceed six inches
at any time.
2. Swales will be inspected monthly or after every runoff producing rainfall event for sediment build-up,
erosion, and trash accumulation.
3. Accumulated sediment and trash will be removed as necessary. Swales will be reseeded or sodded
following sediment removal.
4. Eroded areas of the swales will be repaired and reseeded. Swales will be revegetated as needed and in a
timely manner based on the monthly inspections.
5. Catch basins, curb cuts, velocity reduction devices, and piping will be inspected monthly or after every
significant runoff producing rainfall event. Trash and debris will be cleared away from grates, curb cuts,
velocity reduction devices and piping.
6. Swales will not be altered, piped, or filled in without approval from NCDENR Division of Water Quality.

MEADOWVIEW ELEMENTARY SCHOOL
The Meadowvi.nw Elementary School is proposed to be located on a 30.4 acre
tract at the corner of Pony Farm Road and Firetower Road in Onslow County. The
runoff from this site flows to an unnamed tributary to Blue Creek, which is classified SC
NSW. The project is Iroposed to be built under the Low Density Option of the State's

This office has received a soil erosion and sediment control plan for the project listed above which
was submitted as required by the North Carolina Sedimentation Pollution Control Act (G.S. 113A-57 (4)).
The Act requires that all persons disturbing an area of one or more acres of land must obtain approval
of a soil erosion control plan prior to the commencement of the land -disturbing activity (G.S. 113A-54(d) (4)).
The Act further states that this plan must be filed a minimum of 30 days prior to the activity and the approving
authority must approve or disapprove the submitted plan within 30 days of receipt. Failure of the approving
authority to approve or disapprove the submitted plan within the 30-day period will be deemed approval of the
plan.
Commencement or continuation of a land -disturbing activity under the jurisdiction of this Act prior to
the approval of an erosion and sediment control plan is a violation of the Act.
We have performed a preliminary review of your submitted plan and have determined that additional
information will be necessary before a final review can be completed. Enclosed is a checklist of items that
must be submitted before we can complete the review process. Failure to provide the additional information
may result in disapproval of your plan.
The approval*of an erosion and sediment control plan is conditioned on the applicant's compliance
with Federal and State water quality laws, regulations and rules.

8. Swale details should be site specific. Swales with standing water must be provided
groundcover in the bottom (rip -rap if necessary).
9. Plan shows rock check dam downstream of sediment trap, (ST 6, 12, 15, etc.). Clarify
whether these dams are to be installed in addition to the rock outlet of the trap.

The Wilmington Regional Office received a complete Stormwater Management Permit
Application for Meadowview Elementary School on June 23, 2006. Staff review of the plans
and specifications has determined that the project, as proposed, will comply with the
Stormwater Regulations set forth in Title 15A NCAC 2H.1000. We are forwarding Permit No.
SW8 060347, dated June 27, 2006, for the construction of the subject project.
Please note in particular the operation and maintenance requirements of Section 11.4 of the
permit.
This permit shall be effective from the date of issuance until June 27, 2016 and shall be
subject to the conditions and limitations as specified therein, and does not supersede any
other agency permit that may be required.
If any parts, requirements, or limitations contained in this permit are. unacceptable, you have
the right to request an adjudicatory hearing upon written request within sixty (60) days
following receipt of this permit. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands
are made this permit shall be final and binding.
