STORMWATER DIVISION CODING SHEET POST -CONSTRUCTION PERMITS PERMIT NO. SW 9&5_`oP2 DOC TYPE ❑ CURRENT PERMIT ?v APPROVED PLANS ❑ HISTORICAL FILE ❑ COMPLIANCE EVALUATION INSPECTION DOC DATE YYYYMMDD E 7 0 !MAINTENANCE PLAN 1. All erosion and sediment control practices will be checked for stability and operation following every runoff producing rainfall but in no case less than once every week. Any needed repairs will be made immediately to maintain all practices as designed. 2. All construction entrances will be periodically topdressed with an additional 2 inches of #4 stone to maintlan proper depth. Any sediment that is tracked into the street will be immediately removed. 3. Sediment will be removed from the sediment trap and block and gravel inlet protection device when storage capacity has been approximately 50 % tilled. Gravel will be cleaned or replace when the sediment pool no longer drains properly. 4. Sediment will be removed from behind the sediment fence when it becomes 0.5 feet deep at the fence. The sediment fence will be repaired as necessary to maintain a barrier. 5. The sediment basin will be cleaned out when the level of sediment reaches 2.0 feet below the top of the riser. Gravel will be cleaned or replaced when the sediment pool no longer drains properly. 6. All seeded areas will be fertilized, reseeded as necessary, and mulched according to specifications in the vegetative plan to maintain a vigorous, dense vegetativie cover. STABILIZATION NOTES 1. All slopes will be stabilized within 15 days of cease of any phase of grading. 2. All other areas will be stabilized within 15 days of cease of any phase of grading. CONSTRUCTION SCHEDULE 1. Obtain plan approval and other applicable permits. 2. Hold pre -construction meeting at least one week prior to beginning construction. 3. lnstaR/Maintain temporary construction entrancelexit. 4. Before clearing and grading of the site, install silt fencing at the base of slopes to restrict movement of sediment from the site. 5. Begin clearing and grading of the site. 6. Install storm piping, catch basins, and curb inlets. 7. Install Inlet Protection for catch basins and curb inlets to restrict movement of sediment from the site. 8. All erosion and sediment control practices will be inspected bi-weekly and after each rainfall event. Needed repairs will be made immediately. 9. After the entire covered area is stabilized, an temporary measures will be removed and permanent vegetative repairs to be completed. ti- VEGETATION PLAN 1. After complete stabilization of the entire covered area, top cover soils will be roughened by making 1 =2" deep grooves perpendicular to the slope. 2. Spread time (agricultural limestone) evenly over the area at a rate of 1.5 tans/acre; spread 10--10-10 fertilizer at rate specified and incorporate into the top 6" of soil. 3. Loosen surface prior to applying seed. 4. Broadcast seed at recommended rates. 5. Rake seed into the soil and lightly pack to establish good contact. 6. Mulch with 4000 tb/acre grain straw or equivalent and anchor by weighted farm disc set nearly straight or other mulch anchoring tool. 7. Mow as needed and referti'lize in late winter or early spring by topdressing with 10- 10-10 fertilizer. 8. Graded surfaces shall not be exposed without grass coverage for more than 15 days. If grades are not complete and exposure is more than 15 days, temporary seed as specified. PLACE TEMPORARY SEDIMENT CONTROL MEASURES BELOW THE BASIN AS NEEDED. CONDUIT SPILLWAYS - SECURELY ATTACH THE RISER TO THE BARREL OR BARREL STUB TO MAKE A WATER -TIGHT STRUCTURAL CONNECTIN. SECURE ALL CONNECTIONS BETWEEN BARREL SECTIONS BY APPROVED WATER -TIGHT ASSEMBLIES. PLACE THE BARREL AND RISER ON A FIRM, SMOOTH FOUNDATION OF IMPERVIOUS SOIL. DO NOT USE PERVIOUS MATERIAL SUCH AS SAND, GRAVEL, OR CRUSHED STONE AS BACKFILL AROUND THE PIPE OR ANTI -SEEP COLLARS. PLACE THE FILL MATERIAL AROUND THE PIPE SPILLWAY IN 4 INCH LAYERS AND COMPACT IT UNDER AND AROUND THE PIPE TO AT LEAST THE SAME DENSITY AS THE ADJACENT EMBANKMENT.
