REQUEST FOR ADDITIONAL INFORMATION
Dear Mr. Furr:
DWQ Project # 10-0038
Perquimans County
On February 4, 2010, the Division of Water Quality (DWQ) received your application dated January 18,
2010 to impact 0.08 acres of wetlands to construct a roadway to access a proposed commercial operation
facility at the subject property. The DWQ has determined that your application was incomplete and/or
provided inaccurate information as discussed below. The DWQ will require additional information in
order to process your modified application to impact the streams on the subject property. Therefore,
unless we receive the additional information requested below, we will have to move toward denial of your
application as required by 15A NCAC 2H .0506 and SL 2008-152 and will place this project on hold as
incomplete until we receive this additional information. Please provide the following information so that
we may continue to review your project.
Additional Information Requested:
1. Wetland Impacts
Please provide an account of the additional wetland impacts associated the installation of 18"
RCP culverts and the proposed rip-rap outlet protection as depicted on Drawing C 1.9. Please
note, an account of temporary and permanent wetland impacts will need to be included as part of
the modification.

Please revise the project plans to include an additional wetland impacts.
Please respond in writing to the DWQ within four weeks of the date of this letter with the requested
information. If we do not hear from you within four weeks, we will assume that you no longer want to
pursue this project and we will consider the project withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
authorized (at this time) by the DWQ. Please call Anthony Scarbraugh of the Washington Regional
