The above photo shows the back drainage swale. The photo shows one of three
blockages noticed in the swale. The landscape staff will need to clean out this swale
periodically to ensure the pond will draw down to normal water elevation. The Swale
will need to be periodically cleaned out for proper maintenance.

1. (Quarterly) Cut the brush / vegetation on the berm and side slopes to a
height of approximately G inches.
2. (Yearly) Cut, rake and remove cattail & associated aquatic invasive
weeds when the weeds cover approximately 30% or more of the pond
area or when negatively impacting the function of the pond. Cattails
can quickly become a nuisance if not kept under control.
3. (Monthly) Remove all trash debris and plant matter that builds up on
the concrete outlet control structure.
4. (Monthly) Remove any and all debris and/or obstructing plant material
that builds up in the drainage ditch. This will keep the water flowing
out of the pond area during and after rainfall events.

The photo above shows 1 of 2 inlet storm pipe locations. The inlet is in
passing condition and has recently been cleaned out and is functioning
properly.
The photo above shows the forebay looking south. The forebay is in passing
condition. Previously noticed invasive vegetation has been cut and
removed.

Aquatic vegetation was observed on the pond perimeter as shown above
While aquatic plant species is generally accepted for the purpose of
stabilization. Cattails previously noted have been cut back, raked, and
removed from the area.
The general condition observed at the pond surface shows the absence of
floating algae or other foreign debris. The pond surface appears healthy.

The weir notch is cleared and the back swale out trench has been dredged
and is now flowing. The outlet area is functioning as designed and passes
inspection.

Landscaper's Maintenance Punch List

Attached for your records is the Annual Stormwater BMP inspection report for the SECU
located at 1000 Rhodes Avenue in Burgaw NC. The facility was found to be in passing

Inspection Summary:
The following deficiencies were noted during the inspection:
1. No PE Certification was found in the DWQ File.
2. O&M issues: sediment accumulation in forebay & woody vegetation on pond slopes.
To bring this system into compliance, the following must be completed:
1. Submit a certification of the stormwater system from a PE to this office.
2. Verify the depth of the forebay, remove accumulated sediment if required; remove aquatic vegetation (cattails) to less
than 50% coverage.
3. Remove woody vegetation below the temporary pool level of the pond slopes; re -stabilize pond slopes.
