Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: The project reach spans about 850 linear feet from the beyond upstream limits of the former impoundment of a small dam to about 100 linear feet downstream of the dam spillway. The riser pipe on the dam principal spillway was removed at least two years ago and flow now enters the spillway directly through the culvert barrel. Blockages in the pipe have elevated the water level about 2 feet above the barrel invert, but water level is still at least 5 feet below the original full pool. The stream reach within the former impoundment is incised and unstable. Surrounding land use is low density residential with forested areas. ty 3b. Explain the purpose of the proposed project: The purpose of the project is to improve habitat for native brook trout by removing the dam, restoring the creek, and establishing native riparian buffers. 3e. Describe the overall project in detail, including the type of equipment to be used: The project will involve a dewatering the impoundment, removing the dam embankment, sloping existing channel banks and excavating new channel where appropriate, installing in-stream grade control and habitat enhancement structures and planting native trees and shrubs on the banks and floodplain buffer areas. Conventional construction equipment, including a track mounted excavators, dump trucks and assorted hand tools, will be used to install the project elements. Work will take place from the top of bank as much as is practical. Some equipment access to the creek bottom will be required to install the in-stream structures. Base flow will be diverted around work areas using pumps and hoses. The bottom portion of the dam structure will be left in place at an elevation above the upstream floodplain elevation until all upstream grading is complete and stabilized. 4. Avoidance and Minimization 1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project. The project represents a long-term benefit to the creek because it will restore natural functions to a severly impacted reach, remove a significant fish passage barrier and reduce fine sediment inputs from bank erosion. Impacts will be temporary and minimal. Construction phase best management practices will help minimize the short-term impacts, as described in section 1 b. 1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques. Grading work will begin only after base flow has been diverted around work areas. Work will be staged such that no more area will be disturbed than can be fully stabilized by the end of each work day. Equipment traffic will be limited to designated areas of disturbance and the contractor will be directed to avoid unnecessary traffic in the creek or on adjacent banks. The contract documents will require well maintained and functioning equiptment and daily checks for leaks. An emergency spill plan will also be required. 2.
