Skvarla, 111 Secretary On May 29, 2014, 1 inspected the Hampton Inn development, located at 402 Halstead Boulevard in Elizabeth City to determine compliance with Stormwater Management Permit Number SW7960108, which was issued on October 17, 2006. The DWQ site inspection revealed that the project is in substantial compliance with the approved permit (see note below). Please see attached Inspection report also. • At this point the condition of the wet pond is satisfactory; however some preventive maintenance (i.e, removal of trash, vegetation and sediment) will be needed in near future. Our suggestion would be to undertake some corrective actions to avoid more costly maintenance in future. Thank you for your attention to this matter. Please be advised that you are required to comply with the terms, conditions and limitations of your Stormwater Management Permit under Title 15A North Carolina Administrative Code 2H .1003 and North Carolina General Statute 143-214.7, including operation and maintenance of your permitted stormwater system. Enclosed please find pictures taken during the cleanout of the pond. The pictures show the amount of sediment being removed to return the pond to the design depth. Also included are pictures showing the draw down device and the hole associated with the 1-3/4" draw down orifice. As of this date, all work has been completed and the exposed sides of the pond have been seeded and covered with jute mate. The fountain has been removed from the pond as it is not of adequate size to allow the installation of even a small fountain. Enclosed please find a new wet basin supplement and new application form. Project was designed in 1995 when the forebay was calculated at approximately 20% of the total surface area. e. Pond has been designed for 90% TSS removal. h. Perimeter shelf was not required at the time of permitting. k. The pond is located entirely on the project property and discharges into the NC Department of Transportation piped roadside drainage system. An encroachment agreement was filed with the DOT. Construction on this site was completed approximately nine years ago. The basin was just restored to design depths as part of the permit renewal. Maintenance activities shall be performed as follows: After every significant runoff producing rainfall event and at least monthly: a. Inspect the wet detention basin system for sediment accumulation, erosion, trash accumulation, vegetated cover, and general condition. b. Check and clear the orifice of any obstructions such that drawdown of the temporary pool occurs within 2 to 5 days as designed. 2. Repair eroded areas immediately, re -seed as necessary to maintain good vegetative cover, mow vegetative cover to maintain a maximum height of six inches, and remove trash as needed. 3. Inspect and repair the collection system (i.e. catch basins, piping, swales, riprap, etc.) quarterly to maintain proper functioning. 4. Remove accumulated sediment from the wet detention basin system semi-annually or when depth is reduced to 75% of the original design depth (see diagram below). Removed sediment shall be disposed of in an appropriate manner and shall be handled in a manner that will not adversely impact water quality (i.e. stockpiling near a wet detention basin or stream, etc.). The measuring device used to determine the sediment elevation shall be such that it will give an accurate depth reading and not readily penetrate into accumulated sediments. When the permanent pool depth reads 4 _ ; feet in the main pond, the sediment shall be removed. When the permanent pool depth reads 1. 5 feet in the forebay, the sediment shall be removed. If the basin must be drained for an emergency or to perform maintenance, the flushing of sediment through the emergency drain shall be minimized to the maximum extent practical. Form SWU-102 Rev 3.99 Page 3 of 4 7. All components of the wet detention basin system shall be maintained in good working order.
