A Division of Energy, Mineral, and Land Resources file review has determined that
Stormwater Permit SW7121010 for a stormwater treatment system consisting of a
Storm Filter system serving the Expressway # 4 located on Hwy 70 in Havelock, NC
expires on November 2, 2020. This is a reminder that permit renewal applications are
due 180 days prior to expiration. We do not have a record of receiving a renewal
application.
Please submit a completed permit renewal application along with a $505.00 fee for
permit renewal. Also, it was noted in the file review that a designer's certification that the
project was constructed in accordance with the approved plans as required by the
permit has not been submitted. Please include this with your application. Permit
application forms for renewal can be found on our website at:

On February 4, 2013, the Washington Regional Office received a Plan Revision request for Stormwater
Management Permit Number SW7121010.
The plan revision request is for the following:
• To change the location and orientation of the stormwater detention facility due to a minimum
setback requirements of the onsite septic system.
Based on the current Division of Water Quality Guidance on this issue your plan revision request is hereby
approved. Please replace the previously approved plan set, approved on November 2, 2012 with the revised set
(attached).
Please be aware that all terms and conditions of the permit issued on November 2, 2012 remain in full
force and effect. Please also understand that the approval of this revision to the approved plans for the subject
State Stormwater Permit is done on a case -by -case basis. Any other changes to this project must be submitted
to and approved through the Division of Water Quality prior to construction. The issuance of this plan revision
does not preclude the permittee from complying with all other applicable statutes, rules, regulations or
ordinances which may have jurisdiction over the proposed activity, and obtaining a permit or approval prior to
construction.

Dear Scott,
Please find enclosed two (2) complete sets of revised Engineering drawings for the above
referenced project. As we discussed previously, we need to change the location and orientation
of the stormwater detention facility due to a minimum setback requirement of the onsite septic
system. These changes do not reflect in the calculations, reports, applications, supplements,
elevations, etc. It is more of a plan revision submission than a permit modification. The changes
are strictly and simply a change in horizontal location of the facility. The drawings show the new
layout of the underground detention system and storm filter structure.
Please review and let me know if you have questions or concerns. We would request a letter
accepting the revised system layout as soon as possible as the contractor would like to install the
system, which is today ready to deliver.

Just got your phone message from yesterday. Answers below:
1] SHWT: We have never "called" the SHWT on this site because it was not necessary. The stormwater system as
provided is a completely sealed system. According to my conversations with Contech there wasn't a SHWT
separation requirement for these detention/stormfilter systems.
2) The best I can tell from the calculations provided by Contech, with the filter setup they have the system would
completely drain out in 8 hrs +/-. This is less than two days, however, every bit of the volume would be
completely filtered by media. This is using a flow rate of 0.1 cfs as provided by Contech. If this is an issue I can
have some more conversation with Contech on reworking the filters. i think the faster the box drains the better,
since TSS won't be of concern and since the flow rate of 0.10 cfs doesn't present erosion concerns downstream.
Let me know if you need further information on either of these and I will get the Contech engineers & reps involved.

Dear Scott,
Please find attached the State Stormwater permit application for the above referenced
project. Enclosed items include an (1) original and one (1) copy of the application forms,
two (2) notarized operation and maintenance agreements, two (2) copies of the deeds,
two (2) Department of the Secretary of State printout, two (2) copies of the Engineering
plans, two (2) copies of the Engineering report, and one (1) check for $4,000.00 (Check
No.196) for the express permitting application processing fee.
This project consists of site construction for the purpose of supporting a retail gas service
station facility. The total building area is 3,150 sf +/-. Several gas pumps and diesel
pumps will be installed to serve both passenger vehicles and large trucks. Site work
includes earthwork fill and grading, water and sewer connections for existing services
and septic, tank and underground service line installation, concrete sidewalks and
pavement, building pads, storm drain network, stormwater volume capture and treatment,
and miscellaneous items as typical of this type of construction.

The property herein conveyed does not include the primary residence
of a Grantor

And Grantor covenants with Grantee that Grantor is seized of the premises in fee and has the
