Location/Description of Proposed Activity: The project is located along Poplar Creek and its associated
tributaries. The project area consists of 22.91acres of forests, open fields, wetlands open water, and several
UTs to Poplar Creek. Coordinates of project site in decimal degrees start at 35 79325 N, -78.47196 W, (RR
HEW of way north of Robertson St., SR 2500) and proceed south to 35 73888 N, -78.47029 W (north side of
Grasshopper Rd., SR 2511) along Poplar Creek in the Upper Neuse HUC 03020201 Upper Poplar Creek
Gravity Sewer Line extension consists of approximately 20,000 LF of 8"-30" gravity sewer, and
approximately 4,300 LF of 16" force main. Proposed impacts amount to 1.326 acres of wetland (0.270
permanent, 1.056 temporary), and 598 LF of streams (temporary)

plan. The required plan is either insufficient or has not been included with your notification (see remarks).

implemented to avoid and minimize impacts to Waters of the United States.

Your application/pre-construction notification did not include a delineation of affected special aquatic sites,

REMARKS: Please provide the following information: Please address the temporary/permanent status of
the check dams and the rip-rap in streams proposed on construction drawing numbers: C5, C7, C8, C10,
C 11, C 12, C 14, C 17, and C 18. If temporary, please include construction techniques and any pre-
installation matting and removal techniques. Please address any anticipated maintenance road or access
points post-construction. If any proposed impact amounts change due to the requests from this office or NC
DWQ, please provide an updated PCN with corrected amounts.
If you do not submit the requested information within 30 days from the date of this letter, your
