Dear Mr. Farmer:
The North Carolina Division of Water Resources conducted an inspection of the Twelve Mile Creek
WWTP on October 1, 2020. This inspection was conducted to verify that the facility is operating in
compliance with the conditions and limitations specified in NPDES WW Permit No. NC0085359. The
findings and comments noted during this inspection are provided in the enclosed copy of the inspection
report entitled "Compliance Inspection Report".
Please respond in writing to this office within 30 days of your receipt of this letter regarding your plans
or measures to be taken to address the following issues:
Chain -of -Custody (COC) transportation forms were not complete, signed off by laboratory as received, and not
stored on -site.
Please be advised that in accordance with NPDES Permit # NC0085359, Part II, Section D, For each
measurement or sample taken pursuant to the requirements of this permit, the Permittee shall record the date,
exact place and time of sampling or measurements, and have access to a copy (hard copy or electronic) of any
record that must be kept under the conditions of the permit.
