I received the attached letter regarding the closure of the facility in Roxboro.
Below is a link to the Rescission Request form to be completed regarding Stormwater Certificate of Coverage
NCG210288.
Once completed, please email me a copy and put the original signed form in the mail.
I will begin the workflow sending our regional inspector out.
Rescind an Existing Industrial Permit
Complete the Rescission Request Form and mail one hard copy of the form to Suzanne McCoy, 1612 MSC,
Raleigh, NC 27699-1612. In addition, e-mail the form to Suzanne McCoy. There is no fee for industrial
permit rescissions. Please note that it can take 1-2 months to process the request because we cannot rescind the
permit until DEQ staff conduct a site visit.

Boise Cascade facility located at 1000 North Park Dr., in Roxboro, NC is closed. At this time
there is a security guard at the facility during the day and the facility is visited by a member of
