Dear Mr. Meads:
On February 8, 2007, the Division of Water Quality (DWQ) received your application dated February 5, 2007,
to impact .05 acres of wetlands to construct a proposed residential dwelling. The DWQ has determined that
your application was incomplete and/or provided inaccurate information as discussed below. The DWQ will
require additional information in order to process your application to impact protected wetlands on the subject
property. Therefore, unless we receive the additional information requested below, we will have to move
toward denial of your application as required by 15A NCAC 2H .0506 and will place this project on hold as
incomplete until we receive this additional information. Please provide the following information so that we
may continue to review your project.

1. Project Information
Vicinity Map, Site Map (to scale), USGS Map, and Plan Drawings with North Arrows should be included
with PCN.
2. Complete Name of County, City, and River Basin.
3. Wetland and Upland square footage does not equal to proposed property size (1.58 acres).
4. Prior Project History
Name and Date of Jurisdictional Determination by Army Corp of Engineers (ACE)
5. Impact Justification
No Explanation of Avoidance and Minimization for Wetland Impacts is given.
6. Stormwater
Percent of impervious Surface Proposed Not indicated.

7. Sewage Disposal
Location of Septic Tank, Drain Field, and Repair Area Should Be Indicated on Plan Drawings.
Please respond in writing within three weeks of the date of this letter by sending this information to Mr. Roberto
Scheller of the DWQ Washington Regional Office. If we do not hear from you within three weeks, we will
assume that you no longer want to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at
this time) by the DWQ. Please call Roberto Scheller at 252-948-3940 if you have any questions regarding or
