Mr. Laverty, 

This email is in regards to the Notification of Intent that we submitted to your office on September 8, which had some 
missing information and signatures.  I have attached a copy of the NOI signed in Section K by David Sutton, the 
Rutherford County Schools Superintendent (signature authority for Section C). 

For Section E, there are no preexisting buildings, property boundaries, surface water bodies, water supply wells, septic 
systems, spray irrigation sites, drain fields, repair areas, or sources of groundwater contamination within 250 feet of any 
of the planned geothermal wells.  The tributary to Hollands Creek is approximately 700 feet away to the west at its 
closest approach. 

For Section H, the HVAC/mechanical contractor for the project is Bill Fensterer with Pyatt Heating & Air Conditioning, 
Inc. (PO Box 97, Marion, NC  28752; Phone: (828) 652‐7747; Email: [e-mail]) 

I have also attached our contingency plan for the management of drilling fluids. 

I will be the geothermal project manager for this construction and onsite to supervise all of the work performed by 
Earthtech.  Please contact me with any questions or if you need additional information prior to approval of our NOI.  We 
would like to commence drilling as soon as practicable and would greatly appreciate a quick turnaround on this request.
