As we discussed over the phone earlier, we would like to make
a modification to one of the drainage ditches at Jackey's Creek.
This is a location where the developer merely dumped a large
amount of rip -rack in a bend where erosion has historically
been a problem. The problem continues. We would like to
remedy this situation permanently with a more effective
method. When we talked, you indicated that we should
forward a design sketch to your department. I am forwarding
a copy of the proposed modification for your review and
approval. The design will meet the goal of reducing soil
erosion without impacting the environmental intent of the
permit. We request the department's permission to proceed
with this project. Please contact us at the above address.

Your application for the subject access has been approved. Attached is your copy of the
approved permit subject to the following conditions:
• Installation of 108' of 18" BCCMP and construction of one drop inlet per NCDOT standard
drawing number 840.D15 with one frame & grate per NCDOT standard drawing number 840.29.
• No work shall be allowed within NCDOT right of way between the hours of 6:00 A.M. to
9:00 A.M. and 4:00 P. M. to 7:00 P. M.
Any work requiring equipment or personnel within 5' of the edge of any travel lane shall
require a lane closure with appropriate taper.
Please contact Ms. Iris McCombs, County Maintenance Engineer, at (910) 341-0500
before beginning work. Also, you must call for an inspection Of pipe prior to covering it
Up.

Driveway Access to North Carolina Highways" as adopted by the North Carolina Department of Transportation.
I agree that no signs or objects will be placed on or over the public right-of-way.

conformance with the current "Manual on Uniform Traffic Control Devices for Streets and Highways" and Amendments or
Supplements thereto. Information as to the above rules and regulations may be obtained from the Division Engineer.
I agree to indemnify and save harmless thAorth Carolina Department of Transportation from all damages and claims for damage
that may arise by reason of this construction.
I agree that the North Carolina Department of Transportation will assume no responsibility for any damages that maybe caused to
such facilities, within the highway right-of-way limits, in carrying out its construction.
