None noted in application New expiration date: September 30, 2025 FACILITY OVERVIEW: This facility has two outfalls. The first (001) is 100% domestic waste. The design capacity of the treatment system for this outfall is 0.009 MGD and consists of the following wastewater treatment components: • Two septic tanks • Subsurface sand filter • Tablet chlorine disinfection • Dechlorination equipment • Effluent discharge pipe w/backflow check valve Outfall 002 is a cooling water discharge. No modifications were noted or proposed in the current application. The application noted that flow from outfall 001 (domestic discharge) averaged less than 1 GPD while flow from the cooling water discharge (002) had a monthly average flow of 4,000 GPD. Most recent inspection (04/03/2019) found a few operation and maintenance issues that were pointed out in the inspection report. Overall the facility appears to be in compliance. There have been a few permit violations as outlined in the Compliance section below. The latitude and longitude of the two outfalls in BIMS was showing as the same location as the facility location. This facility location is not accurate and has been updated in BIMS. Based on information in previous renewal applications the outfall locations have been adjusted in the permit and in BIMS as well. COMPLIANCE: There were no enforcement actions during the permit cycle but there have been 8 NOV's and 1 NOD. The facility had five monitoring NOV's for frequency issues for flow, TRC, BOD, TSS and fecal. There were also three NOV's for BOD limit violations as well as one NOD for a late/missing DMR. The region has recently had the permittee do U/D monitoring but found no issues with the results. After discussions with central office staff and ARO staff it was determined that there was insufficient data available to justify the addition of the requirement in the permit. However, as the region has done previously they can continue to look at this issue and require the permittee to do instream monitoring as they did before to assess. What would be needed to add upstream and downstream sampling to this permit? For fecal coliform? This is a very old, unlined, sand filter type system. It basically discharges when the groundwater is elevated, otherwise it is infiltrating into the ground. And given that this system is between two creeks, it is connecting to the streams through that groundwater. This is an old industrial site that sits between two creeks and has historically been bermed in, hence shallow groundwater and the connection comment. Attached are sample results from an event where I had them do U/D fecal sampling, the results were fine.
