Subject: How to Update Your MS4 Permit Info with DEQ
Greetings MS4 Permitted Entities,
Many of you have had staff changes which are not reflected in the DEQ permitting database. Please take a moment to
verify that your permit owner, delegated signatory authorities and billing contact are correct. Just click Stormwater
Permit Summary Report and enter your stormwater permit number (NCS000###). This will generate a form that
includes the address, permit owner, contact(s), billing contact and delegated signature authority on file for the MS4
Permit.
To update any information that is not correct in the Stormwater Permit Summary Report:
• Permit Owner/Legally Responsible Individual - Complete the online Owner Designation/Affiliation Form and
upload the attached signed Permit Owner Affiliation Designation Form.
• Facility address or contact information - Email Suzanne McCoy at [e-mail] and include your
permit number in the email.
• Billing contact - Email Aziza Selkane at [e-mail] and include your permit number in the email.
What does "legally responsible individual' mean? For an MS4 Permit, the responsible party is either: (1) the principle
executive officer or ranking elected official for a municipality, state, federal or other public agency; or, (2) a duly
authorized representative. The legally responsible individual(s) receive all official permit communications.
There is also a separate database for day-to-day MS4 Program Communications contacts, which is used to send out
general communications and information like this email. This database has a primary contact for each permittee,
typically the MS4 Program Administrator, and can include a cc. Consultants and others can also request to be included
on this list. If you received this email, then you are on the MS4 Program Communications list. Emails that are returned
as undeliverable are updated by contacting the MS4 directly. If you have other changes/additions to your MS4 Program
Communications contacts, please just email me the contact email address and I will add them.
