The Division of Water Resources(DWR or the Division)received your request to remit civil penalties in
the subject case on February 18,2020.We have also received your Justification for Remission Request,
with explanation and your signed Waiver of Right to Administrative Hearing and Stipulation of Facts
signed December 7,2019.
The Director has granted 100%remission of this penalty but he must retain costs in the amount of
$108.53. Your payment must be submitted within thirty(30)days of receipt of this letter.Please make

Permittee States:The ORC routinely inspects the canister-style UV bulbs daily,as recommended by the
manufacture,by viewing through the sight glass to verify that the light is on.Upon Notice of Violation,
the Permittee called Shinning Light Electrical Services who performed diagnostics revealing a UV bulb
intensity at 50%capacity,a problem diagnosed as a faulting wiring harness.New parts were ordered and
installed(invoice included with remission request).
DWR Response:DWR concurs that continuing environmental damage was abated. Full
remission recommended.
