Swartley North Carolina Division of Forest Resources Forestry NPS Unit 1616 Mail Service Center Raleigh, NC 27699-1616 Dear Mr. Swartley: On March 8, 2006, the North Carolina Division of Water Quality (DWQ) received the North Carolina Division of Forest Resources' application fora 401 General Water Quality Certification to impact 6501inear feet of an unnamed tributary to Purlear Creek and 2.4 acres of associated herbaceous wetlands, located within the Rendezvous Mountain Educational State Forest in Wilkes County, in order to conduct a stream restoration project. On October 5, 2005, Mr. Daryl Lamb of the DWQ's Winston-Salem Regional Office, accompanied by Ms. Karen Hall and Mr. Mike Shaffer of the North Carolina Stream Restoration Institute, conducted an onsite review of the proposed project. Based on Mr. Lamb's field observations, additional information is required in order that we may determine your project's compliance with 15A NCAC 2H .0500. Please provide five (5) copies of the following information: Please provide a detailed stream restoration plan. At a minimum, this document should address the following topics: 1. Watershed Characterization -General description, drainage area, land use and development potential, water quality, and significant cultural and natural resources including historical resources, archaeological resources, and any rare, threatened, and endangered species and critical habitats. 2. Project Site Existing Conditions -General site description, geology and soils, riparian buffer and natural communities, wetlands, stream characteristics, and constraints. 3. Reference Reach Analysis -Stable stream reach analysis and buffer composition. Stream Restoration Design -Proposed channel description and stream classification, sediment transport, discharge analysis, and structures used for natural channel design. 5. Riparian Buffer Revegetation Design -Eradication of exotic species, wetland enhancement, and planting plan. 6. Monitoring Plan and Success Criteria -Duration and reporting Until the information requested in this letter is provided, I will request (by copy of this letter) that the U.S. Army Corps of Engineers place this project on hold. Also, this project will be placed on hold for DWQ processing due to incomplete information (15A NCAC 2H .0507(h)). The processing of this application will begin when the requested information is received. Please be advised that if your proposed activity is subject to Section 404 of the Clean Water Act (33 U.S.C. 1344), you must also receive a Section 401 Water Quality Certification from the N. C.
