Contractor shall obtain all required permits prior to beginning work 2.0 The existing property lines and the Side Yard Setback shall be located, marked and flagged by a Professional Land Surveyor prior to beginning work. Verify actual setback requirement with the most current Lake Toxaway Community Association Architectural Review Standards. 3.0 Contractor shall verify and establish all Finish Floor Elevations (FFE) prior to beginning work. Do not locate any part of the structure within any set —back, easement, right—of—way, floodway or flood fringe, NCDEQ regulated set —back, trout stream buffer zone (or other buffer zone or set back). Contractor shall obtain all required permits prior to beginning work. Contractor shall locate all property boundaries prior to beginning work. Contractor shall locate, flag and protect all underground utilities prior to beginning work. 4.0 Design criteria and assumptions: Design Method = ASCE-7, Minimum Design Loads for Buildings and other Structures Soil Bearing Capacity = 2000 psf, assumed Floor Live Load = 40 psf Floor Dead Load = 15 psf 5.0 FOOTINGS. The bottom of footings shall be located below the frost line. Footings shall be stepped where it is necessary to change the elevation of the top surface of the footings or where the slope of the bottom surface of the footings will exceed 0.1 unit vertical in ten units horizontal (1% slope). All individual pier footings shall be restrained from sliding with anti —slide keys, helical piers, etc). 6.0 CONCRETE. Concrete shall have a minimum compressive strength of 4000 psi at 28 days and shall have 5 to 7% entrained air. DO NOT ALLOW FRESH CONCRETE TO CONTACT LAKE WATERS. 7.0 REINFORCING STEEL. All reinforcing steel and anchor dowels shall conform to ASTM A 615, Grade 60 and shall have standard deformations. All footer reinforcing bars shall have a minimum of 3" of concrete cover. All rebar lap splices shall be 60 bar diameters unless otherwise noted. All footer steel shall be bonded and grounded per the NEC. 8.0 TROUT BUFFER REQUIREMENTS. Where the "on shore" work is within 25—ft of the top —of —bank, and exceeds 10% of the shoreline frontage, the Contractor shall obtain a Trout Buffer Variance from the NCDEQ—DLR Asheville Regional Office (P.O.C. Stan Akin, 828-296-4500). 15A NCAC 04B .0125 BUFFER ZONE REQUIREMENTS. (a) Unless otherwise provided, the width of a buffer zone is measured from the edge of the water to the nearest edge of the disturbed area, with the 25 percent of the strip nearer the land —disturbing activity containing natural or artificial means of confining visible siltation. (b) The 25 foot minimum width for an undisturbed buffer zone adjacent to designated trout waters shall be measured horizontally from the top of the bank. (c) Where a temporary and minimal disturbance is permitted as an exception by G.S. 113A-57(1), land —disturbing activities in the buffer zone adjacent to designated trout waters shall be limited to a maximum of ten percent of the total length of the buffer zone within the tract to be distributed such that there is not more than 100 linear feet of disturbance in each 1000 linear feet of buffer zone. Larger areas may be disturbed with the written approval of the Director. (d) No land —disturbing activity shall be undertaken within a buffer zone adjacent to designated trout waters that will cause adverse temperature fluctuations, as set forth in 15A NCAC 2B .0211 "Fresh Surface Water Classification and Standards", in these waters. Comply with the latest edition of the Lake Toxaway Community Association Architectural Review Standards. 2.0 CONDITIONS FOR USE OF THIS PLAN. These conditions apply to all sheets and all engineering provided herein. By accepting this Plan, you and any others to whom it is distributed, agree to the following conditions; 2.1 As a result of this design, a number of observations have been made and recommendations provided, including plan(s), details and specifications. It should be noted that the plan(s), detail and specifications provided herein are based on information provided as of this writing. It is conceivable that additional information may be forthcoming which bears on these details and specifications. The right is reserved, therefore, to review and modify all observations, conclusions, details and specifications at any future point in time should, in fact, additional information become available. 2.2 Limitation of Liability. All other work that may be required, but not shown herein, shall be designed by the Contractor. By accepting this engineering document, the Owner, Contractor and any others to whom it is distributed, agree to a strict limitation of liability for TERRY E. ALLEN ENGINEER, Terry E. Allen PE, T.E. ALLEN ENGINEERING, P.C., and its employees to an amount not to exceed $25,000.00 or one—half the total amount of paid invoices, whichever is less. 2.3 Indemnification. The Owner and the Contractor shall indemnify and hold harmless T.E. Allen Engineering, PC (hereinafter the 'Engineer), and its officers, agents, employees and assigns, to the fullest extent permitted by law, for any loss, cost expenses or damages incurred by the Engineer as a result of the Owner's and/or Contractor's breach of this Agreement or for any act or omission caused solely by the negligence of the Owner and/or Contractor, its employees and its other consultants and subcontractors. A 3.0 All work shall be in accordance with North Carolina State Residential Building Code, OSHA Safety and Health Requirements, US Army Corps of Engineers 404 permit requirements, NCDEQ Division of Water Quality 401 permit requirements, NC Sedimentation Pollution Control Act, Lake Toxaway Community Assoc requirements, and all other applicable codes, ordinances, and regulations. The Contractor shall coordinate all work with the Lake Toxaway Operations Manager, 16801 Rosman Hwy, Lake Toxaway, NC 28747. 4.0 TURBIDITY CONTROL. The Contractor shall not violate water quality stream standards during the performance of this work. The Turbidity Level in the lake waters shall not exceed 10 NTUs above background levels at any time during performance of this work pursuant to 15A NCAC 0213.0211 (3)(k). The Contractor shall employ any and all methods necessary to prevent a violation of the Turbidity Standards. As a minimum, the following turbidity control measures shall be employed. These methods shall be modified and improved as necessary to prevent violation of water quality standards; 4.1 To ensure compliance with Water Quality requirements, the work shall be performed "IN THE DRY" using a coffer dam to create a dry work area, or during a Lake Drawdown. Where a coffer dam is used, a minimum of two (2) turbidity curtains shall be used around the outside (wet side) of the coffer dam. The work area isolated by the coffer dam may be dewatered by pumping. All dewatering discharge shall have a turbidity of not more than 10 NTUs above background or as otherwise specified in DWQ's Turbidity Standards (15A NCAC 02B.0211(3.)(k.)). To satisfy the required turbidity limit, the dewatering pump discharge shall be filtered through a Bag Filtration Unit equal to Model BF-400 from Rain —for —Rent with a 100 to 1 micron nominal rating (P.O.C. at Rain —for —Rent is Jeremy at 704-393-3345). The dewatering flow from the flirtation bag shall be discharged in front of (on the work side of) the two (2) turbidity curtains for additional filtration. The turbidity level of all discharge shall be monitored with calibrated turbidity meters and the filtration equipment shall be adjusted as necessary to insure compliance with Water Quality Standards. The water around the work areas shall likewise be monitored with calibrated turbidity meters and the turbidity curtains shall be adjusted as necessary to insure compliance with Water Quality Standards.
