Management Permit Application for the subject property on August 20, 2020. We are forwarding
Certificate of Coverage Number SWG010033, dated September 24, 2020. This certificate of coverage
shall be effective from the date of issuance and shall be subject to the conditions and limitations as
specified therein, and does not supersede any other agency permit that may be required.
