The permits will be effective from the date of issuance for a period of one year and are subject to the conditions and limitations as specified therein.A Well Construction Record shall be completed and signed by the certified drilling contractor for each well.This form shall be submitted to the Department upon completion of the well and shall reference the permit number. When the well is discontinued or abandoned,it shall be abandoned in accordance with ISA NCAC 02C .0113 and a well abandonment record (GW-30) shall be submitted to the Division of Water Resource's Information Processing Unit and the property owner within 30 days of the well abandonment.
