Dear Mr. Pelta,
In accordance with your application received December 2, 2010, with additional
information received on December 9, 2010, we are forwarding herewith Well Construction
Permit No. WM0100242 dated December 9, 2010, issued to Gleason Corporation for the
construction of two (2) monitoring wells as described in the application and attached permit.
These monitoring wells will be drilled on property owned by M Realty LLC (PIN#
965401182600000).
This Permit will be effective from the date of its issuance and shall be subject to the
conditions and limitations as specified therein. Attached is a template Well Construction Record
(GW-la) to be completed and signed by the certified drilling contractor for each well. These
form(s) should be submitted to the Department of Water Quality upon completion of the well(s).

CONSTRUCTION OF A WELL
In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and
other applicable Laws, Rules and Regulations,
PERMISSION IS HEREBY GRANTED TO
Gleason Corporation
FOR THE CONSTRUCTION OF two (2) monitoring wells in the Blue Ridge Geologic Belt, located on
property owner by M Realty LLC (PIN# 965401182600000) in Buncombe County, North Carolina in
accordance with the application dated November 10, 2010, and in conformity with specifications and
supporting data, all of which are filed with the Department of Environment and Natural Resources and
are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions or requirements of
any other applicable laws or regulations.
Construction of wells under this Permit shall be in compliance with the North Carolina Well
Construction Regulations and Standards, and any other State and local laws and regulations pertaining
to well construction.
This Permit will be effective for the construction of monitoring wells from the date of its issuance
and shall be subject to other specified conditions, limitations, or exceptions as follows:
1. You are required to provide a copy of this permit to the property owner;
2. You are required to provide notification to the property owner upon well completion and upon
termination of monitoring; and,
3. The monitoring wells shall be maintained in accordance with 15A NCAC 2C .0108.
