NPDES STORMWATER PERMIT
Two copies of each of the following shall accompany this submittal in order for the application
to be considered complete:
(Do not submit the site Stormwater Pollution Prevention Plan)

RY 1. A current Site Map from the Stormwater Pollution Prevention Plan. The location of
industrial activities (including storage of materials, disposal areas, process areas and
loading and unloading areas), drainage structures, drainage areas for each outfall,
building locations and impervious surfaces should be clearly noted.
RY 2. A summary of Analytical Monitoring results during the term of the existing permit
(if your permit required analytical sampling). Do not submit individual lab reports.
The summary can consist of a table including such items as outfall number,
parameters sampled, lab results, date sampled, and storm event data.
RY 3. A summary of the Visual Monitoring results. Do not submit individual monitoring
reports. The summary can consist of a table including such items as outfall number,
parameters surveyed, observations, and date monitoring conducted.
RY 4. A summary of the Best Management Practices utilized at the permitted facility.
Summary should consist of a short narrative description of each BMP's in place at
the facility. If the implementation of any BMP's is planned, please include
information on these BMP's.
RY 5. A short narrative describing any significant changes in industrial activities at the
permitted facility. Significant changes could include the addition or deletion of
work processes, changes in material handling practices, changes in material storage
practices, and/or changes in the raw materials used by the facility.
RY 6. Certification of the development and implementation of a Stormwater Pollution
Prevention Plan for the permitted facility (Sign and return attached form).
If the final year analytical monitoring of the existing permit term has not been completed
prior to filing the renewal submittal, then the last years monitoring results should be submitted
within 30 days of receipt of the laboratory reports. (i.e. do not withhold renewal submittal
waiting on lab results)

BEST MANAGEMENT PRACTICES

BEST MANAGEMENT PRACTICES FROM THE STORMWATER POLLUTION PREVENTION PLAN
The following BMPs are in place at the CertainTeed facility:
• Secondary Containment — Secondary containment is provided for all storage tanks exposed to
storm water.
• Spill Kits — Spill kits are located near material storage areas including the still yard and the storage
buildings. The spill kits are inspected and material is replaced as needed.
• Inspections — An inspection program is in place for visually inspecting all BMPs. Inspections are
performed frequently on areas where materials are exposed to storm water and for all BMPs.
• Stormwater Retention Wetlands Maintenance Program — Details of the maintenance program for
the four stormwater retention wetlands are provided in the Draft September 2009 Stormwater
Best Management Practices Inspection and Maintenance Plan. The Plan includes specific
requirements for inspecting for clogs and excess debris, maintaining the vegetated areas,
inspecting the structural components, and evaluating the need for component replacement.
• Facility -wide Maintenance Program —The maintenance programs covers electrical equipment,
mechanical equipment, tanks, valves and pipes with potential to cause spills that could enter the
storm water system.
• Proper Storage of Finished Product — Finished product is properly stored with wraps and/or bags.
If wraps and/or bags are torn, finished product will be re -wrapped and/or re -bagged.
• Proper Labeling of Containers — All storage containers for chemicals, petroleum products, and
hazardous wastes are properly labeled for content and hazards.
• Proper disposal of waste materials — Waste disposal areas are identified throughout the site.
Wastes are segregated and disposed in appropriate containers.
• Good Housekeeping — Good housekeeping procedures are implemented to maintain a clean and
orderly work environment.
• Training — All facility personnel receive annual spill prevention briefings that include spill
prevention practices and response procedures.
