To summarize,6 Field Loading reports were updated based on revised lab data(P and K values changed for the fourth quarter). These have been signed by Jay and inserted into the report. 2 Field Loading reports(Site 31,fields 2 and 5)were revised because the third quarter total gallons applied for those mistake. The correction resulted in field 5 total PAN goingdown some(below the 200 limit)and field 2's PAN two fields were reversed by going up some. (S&ME) is submitting all monitoring and reporting requirements as specified in the above referenced permit for CF Industries,Inc. S&ME compiled this revised report using data gathered during the compliance year.This data includes: 1) Certification Form, 2) Residual Sampling Summary Form, 3) 2015 Application Summary and Field Loading Rate Reports,4) Field Application Summaries, 5) Updated Attachment B, 6) Landowner Agreement Attachments &Lessee Agreement Attachments, and 7) Groundwater Report. O Yes ❑ No If no please,provide a written description why the facility was not compliant,the dates,and explain corrective action taken. 1) Only residuals approved for this permit were applied to the permitted sites. 2) Soil pH was adjusted as specified in the permit and lime was applied(if needed)to achieve a soil pH of at least 6.0 or the limit specified in the permit. 3) Annual soils analysis were performed on each site receiving residuals during the past calendar year and three(3)copies of laboratory results are attached. 4) Annual TCLP analysis(if required)was performed and three(3)copies of certified laboratory results are attached. 5) All other monitoring was performed in accordance with the permit and reported during the year as required and three(3) copies of certified laboratory results are attached. 6) The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T.1105(a)or the Pollutant Loading Rates in 15A NCAC 02T.1105(b)(applicable to 40 CFR Part 503 regulated facilities). 7) All general requirements in as specified in the Land Application Permit were complied with(applicable to 40 CFR Part 503 regulated facilities). 8) All monitoring and reporting requirements in 15A NCAC 02T.1111 were complied with(applicable to 40 CFR Part 503 regulated facilities). 9) All operations and maintenance requirements in the permit were complied with or,in the case of a deviation, prior authorization was received from the Division of Water Quality. 10) No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of violations are attached to include appropriate actions and remediations. 11) Vegetative cover was maintained and proper crop management was performed on each site receing residuals,as specified in the permit. 12)No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred. 13) All buffer requirements as specified on the permit were maintained during each application of residuals. Based on flow meter readings between December 27, 2014 and December 31,2015 from Transfer Pump Station #1, approximately 15,306,400 gallons of groundwater were recovered. Land application efforts for 2015, on average, have maintained the groundwater level within the slurry wall at an elevation below 30 feet relative to mean sea level (ft-msl). However, groundwater elevations within the slurry wall area did exceed 30 feet mean sea level (ft. MSL.)on occasion due to restricted land application caused by wet field conditions. Efforts have been made to remove groundwater when weather permits. Groundwater elevations were also measured on a monthly basis during 2015. The monthly groundwater elevation data from December 2014 through December 2015 is presented in Table 1. As required in the permit, potentiometric maps showing quarterly interpretations of general groundwater elevations based on the January,April,July and October 2015 measurements are included as Figures 2, 3,4 and 5. As required by the land application permit, cross-sectional views of water levels measured in piezometers PI-11—P0-12 and PI-2—P0-236 were prepared and are attached as Figures 2A, 3A,4A and 5A for each corresponding quarterly potentiometric map. Additional cross-sectional views of water levels measured in C-designated wells C-23—C-22, C-29—C-28 are included as Figures 2B, 3B,4B and 5B. Cross-sectional views of C-33-C-32 and monitor wells MWI-4—MWI-6 are depicted in Figures 2C, 3C,4C and 5C. The history of water levels for piezometers PI-2, PI-11, P0-12 and P0-236 is graphically shown in Figure 6.The historical water level data from the oldest C-designated wells on site(C-22, C-23, C-28, C-29, C-32 and C- 33) is graphically shown in Figure 7. Historical water level elevation data for MWI-3, MWI-4 and MWI-6 is depicted in Figure 8. Please note that Figures 6, 7 and 8 represent groundwater data collected on a monthly basis representing month to month changes in groundwater elevation, not quarterly changes. Precipitation events were recorded by an on-site weather station/rain gauge. The amounts for 2015 are presented in Table 2. Monthly recorded rainfall on-site ranged from 1.60 inches in May 2015 to a season high of 10.85 inches in July 2015.According to precipitation data from the USA.com website (www.USA.com), the average rainfall amounts for Cofield, North Carolina for May and July are 3.80 inches and 4.80 inches, respectively.
