Please find attached a copy of the completed form entitled "Stormwater Compliance Inspection
Report". The report summarizes the findings of a recent inspection of the project's stormwater controls
conducted on June 172 1997 to determine. compliance with Stormwater Certification Number SW8 921108
issued on September 4, 1996.
A summary of the findings and comments are noted under. Section 6 of the report. Please inform this
Office in writing before July 17, 1997 of the specific actions that will be undertaken and the time frame
required to correct the deficiencies. Failure to provide the requested information, when required, may initiate

On January 15, 1997, this Office copied you on a letter requesting that the new HOA president
provide a signed Operation and Maintenance plan prior to transferring the permit to the Meadowbrook HOA.
To date, that document has not been received in this Office.
I am requesting that you sign the enclosed document and return it to me by May 1, 1997, so that this
file can be cleared up.

responsibility for the subject project on November 25, 1996. In addition to the completed form, the new
owner must also sign an Operation and Maintenance plan and forward the document to me to complete the
