Non -Discharge Compliance Unit:
S&ME, Inc. (S&ME) is submitting all monitoring and reporting requirements as specified in the above
referenced permit for CF Nitrogen, Inc. S&ME compiled this report using data gathered during the
compliance year. This data includes: 1) Certification Form, 2) Residual Sampling Summary Form, 3) 2016
Application Summary and Field Loading Rate Reports, 4) Field Application Summaries, 5) Updated
Attachment B, 6) Landowner Agreement Attachments & Lessee Agreement Attachments, and 7)
Groundwater Report.
CF Nitrogen, Inc. conducted land application events on current permitted fields in Bertie, Hertford, Martin,
and Northampton Counties within the year of 2016. A total of 15,481,000 gallons of nitrogen -enriched
groundwater were land applied.
TCLP, Pathogen and Vector Monitoring and Soil Analysis are not included in the report as these
requirements are not required by Permit due to groundwater recovery system only.

Facility was compliant during calendar year 2016 with all conditions of the land application permit
(including but not limited to items 1-13 below) issued by the Division of Water Quality. © Yes ❑ No
If no please, provide a written description why the facility was not compliant, the dates, and explain corrective action
taken.
I) Only residuals approved For this pernnit were applied to the permitted sites.
2) Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least 6.0 or the
limit specified in the permit.
3) Annual soils analysis were performed on each sire receiving residuals during the past calendar year and three (3) copies
of laboratory results are attached.
4) Annual TCLP analysis (if required) was performed and three (3) copies of certified laboratory results are attached.
5) AI] other monitoring was performed in accordance with the permit and reported during the year as required and three (3)
copies of certified laboratory results are attached.
6) The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T . 1105(a) or the Pollutant
Loading Rates in 15A NCAC 02T .l 105(b) (applicable to 40 CFR Part 503 regulated Facilities).
7) All general requirements in as specified in (he land Application Permit were complied with (applicable to 40 CFR Parl
503 regulated facilities).
8) A11 monitoring and reporting requirements in 15A NCAC 02T .I 1 1 1 were complied with (applicable to 40 C'1•R Part 503
regulated facilities).
9) All operations and maintenance requirements in the permit were complied with or, in the case of a deviation, prior
suthori7ntion was received from the Division of Water Quality.
t 0) No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of violations are
attached to include appropriate actions and remediations.
1 1) Vegetative cover was maintained and proper crop management was performed on each site receing residuals, as specified
in the pennit.
12) No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred.
13) All buffer requirements as specified on the permit were maintained during each application of residuals.

that the water supply well is situated outside of the compliance boundary provided the groundwater standards

On behalf of CF Nitrogen, Inc. (CFN), a wholly -owned subsidiary of CF Industries Holding, Inc. (CFIH),
S&ME, Inc. (S&ME) is submitting the CY 2016 annual groundwater monitoring report for the CFN site
located in Cofield, North Carolina (Figure 1) as required by the Land Application Permit (Permit No.
WQ0012404, with modified date of January 25, 2016). S&ME completed this annual report in general
conformance with CFN Agreement Number (Purchase Order Service) 56101, dated May 18, 2016.
❖ Groundwater Recovery and Distribution Volumes
Weekly flow meter readings were recorded with monthly and quarterly amounts calculated. Based on flow
meter readings recorded from Transfer Pump Station #1 beginning January 1, 2016, and ending
December 28, 2016, approximately 15,865,900 gallons of groundwater were removed from the confined
shallow aquifer in 2016. Quarterly flow amounts for Transfer Pump Station #1 were as follows:
15t Quarter (January — March 2016): 2,487,500 gallons
2nd Quarter (April — June 2016): 3,992,900 gallons
3rd Quarter (July — September 2016): 6,626,200 gallons
41" Ouarter (October — December 2016): 2.759.300 gallons
Total Groundwater Recovery for 2016: 15,865,900 gallons
Based on flow meter readings from Transfer Pump Station #2 and from the Cadman 3250 Reel System,
approximately 15,474,000 gallons of water were transferred from the storage basin for land application.
The nitrogen -enriched groundwater was applied to a variety crops throughout the year, including corn,
cotton, soybeans, wheat, rye and fescue. The following volumes of groundwater may differ slightly to
those reported in the land application report, because meter readings were read on dates when weekly
inspections were conducted, which may have been on different days that were read from the land
application contractor. The flow volumes were recorded during weekly inspections and calculated for
each quarter.

Total Groundwater Applied for 2016: 15,474,000 gallons
Land application of nitrogen -enriched groundwater was approximately 15.5 million gallons during 2016.
Due to an exceptionally high rainy season in the summer and fall of 2016, land application efforts were
not able to maintain the groundwater level within the slurry wall at an elevation below 30 feet relative to
mean sea level (ft-msl). A combination of numerous rain events (Tropical Storm Julia in September and
the occurrence of Hurricane Matthew in October) resulted in approximately 29 inches of rainfall for those
two months. The total annual recorded rainfall at the Ahoskie site was over 69 inches, compared with the
average of 46 inches for this area. This elevated rainfall also resulted in restricted land application caused
by wet field conditions in the latter half of the year. Efforts have been made to remove groundwater
when weather permits.
Groundwater elevations were also measured on a monthly basis during 2016. The monthly groundwater
elevation data from December 20, 2015 through December 28, 2016, are presented in Table 1. As
required in the permit, potentiometric maps showing quarterly interpretations of general groundwater
elevations based on the January, April, July and October 2016 measurements are included as Figures 2, 3,
4 and 5.
In addition, cross -sectional views of water levels measured in piezometers PI-11— PO-12 and PI-2 — PO-
236 were prepared and are attached as Figures 2A, 3A, 4A and 5A for each corresponding quarterly
potentiometric map. Additional cross -sectional views of water levels measured in C-designated wells C-
23 — C-22, C-29 — C-28 are included as Figures 213, 313, 4B and 513, and cross -sectional views of C-33-C-32
and monitor wells MWI-4 — MWI-6 are depicted in Figures 2C, 3C, 4C and 5C. The history of water levels
for piezometers PI-2, PI-11, PO-12 and PO-236 is graphically shown in Figure 6. The historical water level
data from the oldest C-designated wells on site (C-22, C-23, C-28, C-29, C-32 and C-33) are graphically
shown in Figure 7. Historical water level elevation data for MWI-3, MWI-4 and MWI-6 are depicted in
Figure 8. Please note that Figures 6, 7 and 8 represent groundwater data collected on a monthly basis
representing month to month changes in groundwater elevation, not quarterly changes.
Precipitation events were recorded by an on -site weather station/rain gauge. The amounts of
precipitation for CY 2016 are presented in Table 2. Monthly recorded rainfall on -site ranged from 1.99
inches in January 2016 to a season high of 17.09 inches in September 2016. According to precipitation
data from the USA.com website (www.USA.com), the average rainfall amounts for Cofield, North Carolina
for January and September are 3.60 inches and 4.60 inches, respectively. Monthly rainfall for the site, with
a comparison to the average monthly rainfall for Cofield based on data from the onsite weather station, is
presented in Table 2 and depicted in Figure 9.
The amount of rainfall in 2016 was 20 inches above the annual average rainfall for the region. This
quantity of rainfall hindered land application due to permit conditions III.7.d and III.7.f. Given the
exceedingly high amount of precipitation in 2016 and the permit limitations of when groundwater can be
land applied, the elevation within the slurry -wall confined area at the CFN site has increased. The land
