(Nova) prepared this Active Remediation Monitoring Report on behalf of Gaston County Dyeing Machine (GCDM). This report summarizes current treatment system operational data obtained on October 15, 2015. The site is located in the town of Stanley, Gaston County, North Carolina. Figure 1 is a Topographic Map of the property and surrounding area. Nova prepared this report in general accordance with the North Carolina Department of Environment and Natural Resources (NCDENR), Division of Environmental Management (DEM), Non -Discharge Permit Number W00008907. The permit requires monitoring wells MW-5, MW-7 (formerly MW-713), MW-14, and MW-16 to be sampled in October of each year. Please note that MW-7B (intermediate depth well) will be referred to as MW-7 in accordance with the NCDENR Permits and Compliance Unit. Per the July 5, 2013, letter from NCDENR, the groundwater Remediation Permit No. WQ0008907 has been renewed with modifications and will now expire on November 30, 2018. In accordance with the new permit, GCDM was required to permanently abandon the former recovery well RW-7 and monitoring well MW-3. On November 26, 2013, Geologic Exploration completed the well closures on behalf of GCDMC. Figure 2 is a Site Map detailing pertinent site features and monitoring well locations. RH15-5986 PAGE 1 NOVA CONSULTING Nova measured the depth to the water table in the on -site and off -site monitoring wells on October 15, 2015. The historical static water level measurements and elevation data are presented in Table 1. The static water level measurements at this site indicate the direction of ground water flow is generally toward the west and southwest. Figure 3 is a Potentiometric Map that illustrates the direction of groundwater flow. Nova collected water samples from monitoring wells MW-5, MW-7, MW-14, and MW-16 on October 15, 2015. Nova submitted the groundwater samples to Pace Analytical Services, Inc. (Pace) for laboratory analyses for standard pH and volatile organic compounds (VOCs) by EPA Methods 9040 and 624, respectively. According to the laboratory analytical report, tetrachloroethene (PCE) was detected at concentrations of 11.3 micrograms per liter (ug/L), 5.7 ug/L and 4.5 ug/L in the groundwater samples that were collected from monitoring wells MW-7, MW-14, and MW-16, respectively. A summary of the most recent and historical groundwater analytical results are included in Table 2. The laboratory report for the 2015 sampling event is included in Appendix A. Figure 4 is a PCE Concentration Map from the October 15, 2014, sampling event. The PCE concentrations in wells Mw-7, MW-14 and MW-16 remain steady with slight flucuations year after year. In general, the remaining concentrations observed in the groundwater are at trace levels and are slowly degrading and diluting. See Table 2 and Figure 4 for the PCE concentrations. Nova completed the required GW-59 forms for each well and will submitted them to the DENR Groundwater Section, Permits and Compliance Units, as required by the non -discharge permit. Copies of the GW-59 forms are included in Appendix B. GASTON COUNTY DYEING MACHINE STANLEY FACILITY NOVA PROJECT NO. RHIS-5986 PAGE 2 NOVA CONSULTING 3.1 System Sampling Nova collected samples from the influent, before the carbon canisters, between the carbon canisters (carbon), and from the effluent on October 15, 2015. The samples were submitted to Pace for laboratory analysis of nitrates, standard pH, and VOCs in accordance with EPA Methods 352.2, 9040, and 624. The influent, between carbon and effluent samples showed no detectable VOC concentrations. The pH results showed the water to be within the recommended 6.2-8.5 range and the nitrate analytical results showed the concentrations below the regulatory limit of 10 milligrams per liter (mg/L). Per the permit, nitrates will no longer be analyzed as part of the future groundwater sampling. Table 3 presents the historic influent and effluent sample analytical results. The laboratory analytical report is provided in Appendix A. 3.2 Operation and Maintenance Between October 22, 2014, and October 15, 2015, the remediation system operated an estimated 343 days. During the reporting period, approximately 289,971 gallons of water were treated by the remediation system. The total quantity of water that has been treated since remediation commenced at this site is 7,784,283 gallons. The average flow rate for this period was approximately 845.4 gallons per day. The effluent water was injected at an average rate of approximately 0.59 gallons per minute with the injection well pressure readings generally ranged between 2.0 and 5.0 pounds per square inch (psi). The pressure is below the injection well permit maximum of 5.0 psi. During the October sampling event it was determined that the carbon was functioning as designed. RH15-5986 PAGE 3 NO'VO c munNe Tetrachloroethene (PCE) has historically been detected at concentrations exceeding state standards in groundwater samples that have been collected at the site. However, these concentrations have reduced over time in the area of impact near the former lagoon. Recent sampling indicated PCE concentrations of 11.3 ug/L in MW-7 (formerly MW-713), and 5.7 ug/L in MW-14, and 4.5 ug/L in MW-16. No impact was detected of PCE in MW-5 or the influent sample from RW-1. The geometric plume shape has remained fairly stationary and the contaminant concentrations have been reduced through a combination of natural attenuation and removal by the remediation system. The core of the plume has reduced in size and concentration to only trace levels since the remediation system was activated. In the most down -gradient well (MW-16), I PCE concentrations are starting to fluctuate between below laboratory detection limits and trace levels just above the laboratory detection limits. During the October 15, 2015, sampling event no concentrations of contaminates were detected in the influent, between carbons or effluent samples. GCDM will continue to operate the remediation system until the plume boundaries can be confirmed over time and PCE is reduced sufficiently to justify turning off the remediation system. GCDM will conduct the next monitoring event in October 2016 per the permit requirements. The current NPDES permit expires on November 30, 2018. GCDM will submit a renewal application in May 2018.
