RH16-5393 PAGE i NO4 wiurw Nova Consulting Group, Inc. (Nova) prepared this Active Remediation Monitoring Report on behalf of Gaston County Dyeing Machine (GCDM). This report summarizes current treatment system operational data obtained on October 27, 2016. The site is located in the town of Stanley, Gaston County, North Carolina. Figure 1 is a Topographic Map of the property and surrounding area. Nova prepared this report in general accordance with the North Carolina Department of Environment and Natural Resources (NCDENR), Division of Environmental Management (DEM), Non-Discharge Permit Number WQ0008907. The permit requires monitoring wells MW-5, MW-7 (formerly MW-7B), MW-14, and MW-16 to be sampled in October of each year. Please note that MW-7B (intermediate depth well) will be referred to as MW-7 in accordance with the NCDENR Permits and Compliance Unit. Per the July 5, 2013, letter from NCDENR, the groundwater Remediation Permit No. WQ0008907 has been renewed with modifications and as of this report the NPDES permit was rescinded (Per GCDM request) on June 2, 2016. In accordance with the new permit, GCDM was required to permanently abandon the former recovery well RW-7 and monitoring well MW-3. On November 26, 2013, Geologic Exploration completed the well closures on behalf of GCDMC. Figure 2 is a Site Map detailing pertinent site features and monitoring well locations. GCDM had requested to terminate the remediation system and it was granted on June 2, 2016. As part of the decommissioning of the remediation system GCDM was required to have the injection well properly abandoned by a certified well driller, and disassembly of the remediation system which included; the removal of the pneumatic pumps and controls, removal of the well pump from RW-1, the removal of the activated carbon filtration system, and disposal of the carbon at an approved disposal facility. Nova completed the well abandonment on October 27, 2016 as well as removed the pump from the well and removed the carbon from the low-pressure carbon vessels for characterization and disposal. Nova had collected the Carbon samples on September 9, 2016, and had them analyzed for eight RCRA metals and volatile organic compounds per the toxicity characteristic leaching procedure by PACE Laboratories. The laboratory results showed the Spent Carbon to be non-hazardous. As of the writing of the report the waste carbon has not been disposed, however, GCDM will be disposing of it within the next month through Giant Resource Recovery. Upon completion of the disposal Nova will provide a copy of the waste manifest to the North Carolina Department of Natural Resources as proof of the disposal. The decommissioning report will be filed under a separate cover. The remainder of the remediation equipment will be disposed of or recycled as appropriate. The remainder of the monitoring wells will remain active until the Inactive Hazardous Waste Division issues a no further action letter on the site. RH16-5393 PAGE 1 NOV CONSUL7?NC 10 ANNUAL AL l UNDW O I Nova measured the depth to the water table in the on-site and off-site monitoring wells on October 27, 2016. The historical static water level measurements and elevation data are presented in Table 1. The static water level measurements at this site indicate the direction of ground water flow is generally toward the west and southwest. Figure 3 is a Potentiometric Map that illustrates the direction of groundwater flow. Nova collected water samples from monitoring wells MW-5, MW-7, MW-14, and MW-16 on October 27, 2016. Nova submitted the groundwater samples to Pace Analytical Services, Inc. (Pace) for laboratory analyses for standard pH and volatile organic compounds (VOCs) by EPA Methods 9040 and 624, respectively. Per the laboratory analytical report, tetrachloroethene (PCE) was detected at concentrations of 12.9 micrograms per liter (ug/L), 5.5 ug/L and 4.0 ug/L in the groundwater samples that were collected from monitoring wells MW-7, MW-14, and MW-16, respectively. A summary of the most recent and historical groundwater analytical results is included in Table 2. The laboratory report for the 2016 sampling event is included in Appendix A. Figure 4 is a PCE Concentration Map from the October 27, 2016, sampling event. The PCE concentrations in wells MW-7, MW-14 and MW-16 remain steady with slight fluctuations year over year. In general, the remaining concentrations observed in the groundwater are at trace levels and are slowly degrading and diluting. See Table 2 and Figure 4 for the PCE concentrations. Nova completed the required GW-59 forms for each well and will submitted them to the DENR Groundwater Section, Permits and Compliance Units, as required by the non-discharge permit. RH16-5393 PAGE 2 NOVklivr The remediation system went down in October 2015 due to the air compressor had seased working. Nova on behalf of GCDM began the dialogue with the inactive Hazardous Waste Branch in December 2015 to turn the remediation system off since the source wells showed the source area had been remediated to below detection limits and the remaining plume is being degraded through natural attenuation. Based on those Conversations Nova and GCDM pursued the request to terminate NPDES Permit and decommission the remediation system. GCDM received a letter dated June 2, 2016 from the NCDENR approving the termination of the NPDES permit and decommission the system. Therefore, the system has not operated since the October 2015 sampling event and subsequently did not sample the system. During the system operation period, approximately 7,784,283 gallons had been treated and re-injected back into the aquifer. The average flow rate was approximately 845.4 gallons per day. The effluent water was injected at an average rate of approximately 0.59 gallons per minute with the injection well pressure readings generally ranged between 2.0 and 5.0 pounds per square inch (psi). RH16-5393 PAGE 3 -1111k- NOVA CONSULT rG 01 1 1 f Alp • « V V 1 . 1 Tetrachloroethene (PCE) has historically been detected at concentrations exceeding state standards in groundwater samples that have been collected at the site. However, these concentrations have reduced over time to below detection limit in the source area. The remaining contaminant plume down gradient of the source area has only trace concentrations of PCE remaining. Recent sampling indicated PCE concentrations of 12.9 ug/L in MW-7 (formerly MW-7B), and 5.5 ug/L in MW-14, and 4.0 ug/L in MW-16. No impact was detected of PCE in MW-5. The geometric plume shape has remained stationary and the contaminant concentrations have been reduced through a combination of natural attenuation and removal by the remediation system. The core of the plume has reduced in size and concentration to only trace levels since the remediation system was activated. The NPDES permit was rescinded as of June 2, 2016, and the system decommissioning was completed on October 27, 2017 which included the removal of the pneumatic pump in RW-1, the abandonment and closure of the injection well and the removal of the carbon filtration system. The historic Influent and effluent sample results are included in Table 3 and show that the source area has been remediated to below detection limits. GCDM will continue to conduct annual groundwater sampling of the 4 monitoring wells until the PCA concentrations reduce to below the regulatory limit and closure can be obtained.
