NDWRF Reclaimed Water Bulk Distribution Permit Modification
To Whom It May Concern,
The City of Durham is submitting a Permit Application for a minor modification to Permit
Number WQ0032515 for the North Durham WRF Reclaimed Water Bulk Distribution System. The City
of Durham is requesting the modification to allow off -site distribution for the use of Toilet Flushing at the
Mist Lake Facilities. The distribution line from North Durham WRF to Mist Lake has been permitted
with Permit Number WQ0040865.
Modifications requested to the subject permit are as follows:
Off -site distribution of Reclaimed Water to Mist Lake for toilet flushing added to
Attachment D — Approved Bulk Fill Reclaimed Water Uses. Total estimated Reclaimed
Water distribution of 5,400 GPD for toilet flushing.
As the City of Durham has a current permit for the Reclaimed Water Distribution System and is
only requesting to allow distribution for toilet flushing off -site, Contract Drawings and Site Plans have
not been provided. As the equipment has previously been permitted in WQ0032515, the equipment was
not added in RWPI 06-16 and RWBD 06-16 to prevent incorrect equipment identification, and equipment
being permitted multiple times. Please refer to current Permit Number W0032515 for the previous items.

15A NCAC 02U — RECLAIMED WATER SYSTEMS -- PROJECT INFORMATION
INSTRUCTIONS FOR FORM: RWPI 06-16 & SUPPORTING DOCUMENTATION
Please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these
instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of additional
information requested. Failure to submit all of the required items will lead to additional processing and review time for the
permit application.

General —This form is required for all projects involving the generation, utilization, and or distribution of reclaimed water. At least one
of the following forms must also be included in the application package. However, multiple fortes may be applicable. Unless
otherwise noted, the Applicant shall submit one original and two copies of the application and supporting
documentation. Please check the boxes below as appropriate to indicate which additional forms are included in the
application package:
❑ Reclaimed Water Generation (FORM: RWG) — for wastewater treatment facilities producing reclaimed water.
❑ Non -Conjunctive Utilization (FORM: RWNC) — for reclaimed water utilization activities, when the reclaimed water
utilization activity is required in order to meet the wastewater disposal needs of the facility.
❑ Conjunctive Utilization (FORM: RWCU) — for reclaimed water utilization activities, when the reclaimed water option is
not necessary to meet the wastewater disposal needs of the facility and other wastewater utilization/disposal methods are
available.
® Bulk Distribution (FORM: RWBD) — for distribution of reclaimed water in a bulk manner.
❑ Distribution Lines (FORM: RWDL) — for construction of reclaimed water distribution lines.
❑ Wetland Augmentation (FORM: RWWA) — for projects seeking to utilize reclaimed water for the purpose of wetland
augmentation.
❑ Local Program Approval (FORM: RWLPA) — for projects seeking permitting delegation for reclaimed water users.

A major modification shall be defined as any permit modification that: increases the generating facility's
average daily flow (ADF); increases the utilization area acreage; adds additional utilization areas not
previously approved; or includes the addition of new treatment units/processes not previously permitted.
There is no fee for minor permit modifications.

® Submit the completed and appropriately executed Reclaimed Water Project Information (FORM: RWPI 06-16) form. Please
do not make any unauthorized content changes to this form.
® The applicant name in Item 1.1. shall be consistent with the applicant name on the plans, specifications, agreements, etc.
® The Applicant's Certification on Page 3 of this form shall be signed in accordance with 15A NCAC 02T .0106(b). The
application must be signed by a principal executive officer of at least the level of vice-president or his authorized representative
for a corporation; by a general partner for a partnership or limited partnership; by the proprietor for a sole proprietorship;
and by either an executive officer, an elected official in the highest level of elected office, or other authorized employee for a
municipal, state, or other public entity. An alternate person may be designated as the signing official if a delegation letter is
provided from a person who meets the criteria in l 5A NCAC 02T .0106(b).

offsite utilization. Unless otherwise noted, the Applicant shall submit one. original and two copies of the application
and supporting documentation.

if the source facility of the reclaimed water to be distributed is not already permitted for generation of reclaimed water, then
the Reclaimed Water Generation application (FORM: RWG) must also be submitted.
A. Bulk Distribution of Reclaimed Water (FORM: R*VBD 11-13) Application (All application packages):
® Submit the completed and appropriately executed Bulk Distribution of Reclaimed Water form (FORM: RWBD 06-16). Please
do not make any unauthorized content changes to this form. If necessary for clarity or due to space restrictions, attachments to
the application may be made, as long as the attachments are numbered to correspond to the section and item to which they
refer.
® The facility name in Item II.1. shall be consistent with the user name on the plans, specifications, agreements, etc.
® The Professional Engineer's Certification on Page 4 of this form shall be signed, sealed and dated by a North Carolina licensed
Professional Engineer.
® The Applicant's Certification on Page 4 of this form shall be signed in accordance with 15A NCAC 02T .01%(b). The
application must be signed by a principal executive officer of at least the level of vice-president or his authorized representative
for a corporation; by a general partner for a partnership or limited partnership; by the proprietor for a sole proprietorship;

❑ Plan and profile views of all onsite reclaimed water storage units to be used for bulk distribution.
❑ Location and details of all reclaimed water bulk loading stations.
❑ Location and details of all distribution equipment such as pumps, flow meters, turbidity meters, etc.
❑ Plans shall represent a completed design and not be labeled with preliminary phrases (e.g., FOR REVIEW ONLY, NOT
FOR CONSTRUCTION, etc.) that indicate they are anything other than final specifications. However, the plans may be
labeled with the phrase: FINAL DESIGN - NOT RELEASED FOR CONSTRUCTION.
C. Site Map (All Application Packages):
❑ Per 15A NCAC_ 02U .0201Id), submit standard size and 11 x 17-inch site maps that have been signed, sealed and dated by a

reclaimed water. At a minimum, the description should include the following items:
❑ Educational material to be provided to users of the reclaimed water, and a description of the training process.
❑ Educational material should include a reclaimed water definition, allowable uses, prohibited uses, guidance on proper use
(i.e., no runoff, setbacks), and safety/hygiene issues.
❑ Procedures for spill reporting.
❑ Process and documentation for approving users of bulk reclaimed water.
❑ Where necessary, bilingual material shall be provided to non-English speaking communities.

herewith Permit No. WQ0032515 dated December 28, 2016, to the City of Durham for the continued
operation of the subject reclaimed water bulk distribution system.
Modifications to the subject permit are as follows:
➢ Your permit's format has been updated to be consistent with the most current reclaimed water
permit template, and rule references have been updated pursuant to the new 15A NCAC 02U
rules that became effective July 18, 2011.
➢ The two 5,000 gallon reclaimed water storage tanks and manual chlorine dosage system have
been removed and replaced by a hypochlorite chlorination system.
This permit shall be effective from the date of issuance until November 30, 2021, shall void Permit
No. WQ0032515 issued March 7, 2008, and shall be subject to the conditions and limitations as specified
therein. Please pay particular attention to the monitoring requirements listed in Attachments A and D for
they may differ from the previous permit issuance. Failure to establish an adequate system for collecting
and maintaining the required operational information shall result in future compliance problems.

amended, and other applicable Laws, Rules and Regulations
PERMISSION IS HEREBY GRANTED TO

continued operation of a reclaimed water bulk fill station consisting of:
two variable frequency drive (VFD) pumps to maintain pressure to the distribution system; a 4-inch
overhead loading arm; a 3-inch quick connect fitting at ground level for distribution to customers; a
hypochlorite chlorination system consisting of a sodium hypochlorite bulk tank with meter pump to supply
chlorine to the pump feed system; a manual valve at the loading station with a turbidimeter, flow meter and
residual chlorine monitor; a catch basin that drains to the headworks; and all associated piping, valves,
controls, and appurtenances;
to serve the North Durham WRF, with no discharge of wastes to surface waters, pursuant to the application
received April 29, 2016, and in conformity with the project plans, specifications, and other supporting data
subsequently filed and approved by the Department of Environmental Quality and considered a part of this
permit.
This permit shall be effective from the date of issuance until EXPIRATION DATE, shall void Permit No.
WQ0032515 issued March 7, 2008, and shall be subject to the following specified conditions and
limitations:
I. SCHEDULES
No later than six months prior to the expiration of this permit, the Permittee shall request renewal of
this permit on official Division forms. Upon receipt of the request, the Division will review the
adequacy of the facilities described therein, and if warranted, will renew the permit for such period of
time and under such conditions and limitations as it may deem appropriate. Please note Rule 15A
NCAC 02T .0105(d) requires an updated site map to be submitted with the permit renewal application.

H. PERFORMANCE STANDARDS
The subject reclaimed water facilities shall be effectively maintained and operated at all times so there
is no discharge to surface waters, nor any contravention of groundwater or surface water standards. In
the event the facilities fail to perform satisfactorily, including the creation of nuisance conditions due
to improper operation and maintenance, or failure of the utilization areas to adequately assimilate the
reclaimed water, the Permittee shall take immediate corrective actions including Division required
actions, such as the construction of additional or replacement reclaimed water distribution facilities, or
cessation of reclaimed water utilization activities. [G.S. 143-215.1, 143-213.3(a)]
2. This permit shall not relieve the Permittee of their responsibility for damages to groundwater or surface
water resulting from the operation of this facility. [15A NCAC 02B .0200, 02L .0100]
3. Effluent limitations for generated reclaimed water shall not exceed those specified in Attachment A.

4. Reclaimed water distributed through bulk fill stations may only be utilized for the purposes listed in
Attachment D. [G.S. 143-215.1 ]
5. The conjunctive use of reclaimed water permitted herein in no way negates, precludes, or invalidates
the most recent reissuance of the Permittee's NPDES permit (Permit No. NC0023841), and the
Permittee shall continue to comply with all conditions provided for therein. [G.S. 143-215.1]
6. The following shall be requirements for the reclaimed water distribution facilities:
a. All reclaimed water valves, storage facilities and outlets shall be tagged or labeled to warn the
public or employees that reclaimed water is not intended for drinking. Where appropriate, such
warning shall inform the public or employees to avoid contact with reclaimed water.
b. All reclaimed water piping, valves, outlets and other appurtenances shall be color -coded, taped or
otherwise marked to identify the source of the water as being reclaimed water.
i. All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone 522)
and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER — DO
NOT DRINK" or be installed with a purple (i.e., Pantone 522) identification tape or
polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and
repeated every three feet or less.
ii. Identification tape shall be at least three inches wide and have white or black lettering on purple
(i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER — DO NOT DRINK."
Identification tape shall be installed on reclaimed water pipelines in a visible manner, fastened
at least every 10 feet to each pipe length and run continuously the entire length of the pipe.
iii. Existing underground distribution systems retrofitted for the purpose of distributing reclaimed
water shall be taped or otherwise identified as noted above. This identification need not extend
the entire length of the distribution system, but shall be incorporated within 10 feet of crossing
any potable water supply line or sanitary sewer line.
c. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits
operation by authorized personnel only.
d. Hose bibs shall be located in locked, below grade vaults that shall be labeled as being of non -
potable quality. As an alternative to the use of locked vaults with standard hose bib services, other
locking mechanisms such as hose bibs which can only be operated by a tool may be placed above
ground and labeled as non -potable water.

7. No direct cross -connections shall be allowed between reclaimed water and potable water systems,
unless such connection has been approved by the Department pursuant to 15A NCAC 18C .0406. [15A

8. The facilities permitted herein shall be constructed according to the following setbacks:
a. The setbacks for reclaimed utilization sites shall be as follows (all distances in feet):
i. Surface waters not classified SA: 25
ii. Surface waters classified SA: 100
iii. Any well with exception to monitoring wells: 100

I1I. OPERATION AND MAINTENANCE REQUIREMENTS
1. The reclaimed water utilization facilities shall be properly maintained and operated at all times. The
facilities shall be effectively maintained and operated as a reclaimed water system to prevent the
discharge of any reclaimed water or partially treated effluent resulting from the operation of this facility.

2. The Permittee shall maintain an Operation and Maintenance Plan, which at a minimum shall include
the following:
a. Description of the system in sufficient detail to show what operations are necessary for the system
to function and by whom the functions will be conducted;
b. A map of all distribution lines and record drawings of all utilization systems under the Permittee's
control;
c. Description of anticipated maintenance activities;
d. Include provisions for safety measures including restriction of access to sites and equipment; and
e. Spill control provisions including response to upsets and bypasses including control, containment,
remediation, and contact information for plant personnel, emergency responders and regulatory
agencies.
f. For bulk fill operations, the Permittee shall develop an operation and maintenance plan for bulk
distribution activities which includes but is not limited to the following:
i. Description of operation;
ii. Description of anticipated maintenance;
iii. Safety measures, including site restrictions;
iv. Spill prevention and spill response plan;
v. Contact information for plan personnel, emergency responders, and regulatory agencies;
vi. Approval procedures of users;
vii. Operational requirements for users; and
viii.If reclaimed water is to be stored for greater than 72 hours prior to use, included a description
of how compliance with reclaimed water quality standards will be ensured the time of use.

Upon the Water Pollution Control System Operators Certification Commission's (WPCSOCC)
classification of the subject non -discharge facilities, in accordance with 15A NCAC 08G .0200 the
Permittee shall designate and employ a certified operator in responsible charge (ORC) and one or more
certified operator(s) as back-up ORC(s). The ORC or their back-up shall visit the facilities in
accordance with 15A NCAC 08G .0200, and shall comply with all other conditions specified in the
previously cited rules. [15A NCAC 02U .0117]
4. The turbidimeter shall be tested and calibrated at a minimum of once per year. Calibration records
shall be maintained at the facility for a period -of no less than five years, and shall be made available to
the Division upon request. [15A NCAC 02T .0108(b)(1)]

5. Only reclaimed water generated at North Durham WRF (Permit No. NC0023841) shall be distributed
in accordance with this permit. [G.S. 143-215.1]
6. Effluent shall be discharged in accordance with Permit No. NC0023841 if the limit for fecal coliform
(e.g., daily maximum concentration of 25 colonies per 100 mL) or turbidity (e.g., instantaneous
maximum of 10 NTU) is exceeded, until such time as the reclaimed water standards are met at the
generating facility. [15A NCAC 02U .0401(c)]
7. The Permittee shall provide notification to the public and/or employees about the use of reclaimed
water, and that reclaimed water is not intended for drinking. Such notification shall be provided to
employees in a language they can understand. [15A NCAC 02U .0501]
8. Reclaimed water used for activities other than land application shall not be used in a manner that causes
exposure to aerosols. [15A NCAC 02U .0501(b)(6)]
9. Reclaimed water shall not be used for swimming pools, hot tubs, spas, or similar uses. [15A NCAC
02U .0501(d)]
10. Reclaimed water shall not be used for direct reuse as a potable water supply. [15A NCAC 02U
.0501(e)]
11. Bulk distribution operations shall meet the following:
a. The Permittee shall approve tank truck drivers for participation in the bulk reclaimed water
distribution program prior to distribution of reclaimed water. In addition, the Permittee shall
provide bilingual (English and Spanish) training to all tank truck drivers approved to participate in
the bulk reclaimed water distribution program at a regular frequency. These sessions shall provide
training on, but not limited to, the following topics:
i. Reclaimed water description and safety;
ii. Allowable and prohibited uses of reclaimed water;
iii. Procedures for the application (including buffer maintenance), uses, and storage of reclaimed
water as well as the disposal of unused reclaimed water;
iv. Procedures for spill prevention, countermeasures, and control as well as notification
requirements;
v. Personal safety and hygiene issues; and
vi. Interaction with the general public.
b. Tank trucks used to transport reclaimed water shall meet the following requirements:
i. Tank trucks and other equipment used to distribute reclaimed water shall be clearly identified
with advisory signs. Signage may include but is not limited to "CAUTION: RECLAIMED
WATER — DO NOT DRINK", "NON -POTABLE WATER", or a combination of similar
phrases.
ii. Tank trucks used to transport reclaimed water shall not be used to transport potable water that
is used for drinking or other potable purposes.
iii. Tank trucks used to transport reclaimed water shall not be filled through on -board piping or
removable hoses that may subsequently be used to fill tanks with water from a potable water
supply. Tank trucks owned and operated by the Permittee and dedicated to be filled by
reclaimed water only may be filled through on -board piping.
iv. Tank trucks shall not have been used to haul septage, biosolids, or other waste or chemical
transport.
v. Tanks trucks shall not have any type of removable container or portable bladder except if the
tank/bladder is permanently labeled for reclaimed water use only and a letter from the truck
owner is submitted to the Permittee indicating that in no case shall any removable container or
portable bladder be used for both reclaimed and potable water at any time.

IV. MONITORING AND REPORTING REQUIRMENTS
1. Any Division required monitoring (including groundwater, plant tissue, soil and surface water analyses)
necessary to ensure groundwater and surface water protection shall be established, and an acceptable
sampling reporting schedule shall be followed. [15A NCAC 02T .0108(c)]
2. A Division certified laboratory shall conduct all laboratory analyses for the required effluent,
groundwater or surface water parameters. [15A NCAC 02H .0800]
3. The Permittee shall monitor the reclaimed water from the generating facility at the frequencies and
locations for the parameters specified in Attachment A. [15A NCAC 02T .0108(c)]
4. The Permittee shall maintain adequate records for the bulk reclaimed water distribution program.
Records shall be maintained for a minimum of five years. These records shall include, but are not
necessarily limited to, the following information:
a. Date and time of bulk distribution event,
b. Name of receiver or town -issued bulk fill certification number,
c. Intended use of the bulk reclaimed water,
d. Number of gallons provided at each bulk distribution event, and
e. Total volume of bulk reclaimed water distributed each month.
Monthly tracking records for bulk distribution shall be summed and reported Form NDMR (see
Attachment A) under parameter WQO1 (Flow, Reclaimed Water Distributed). This value shall
represent the total volume of reclaimed water distributed via bulk fill for that month. [ 15A NCAC 02T
.0108(c)]
5. A maintenance log shall be maintained at this facility. This log shall be maintained at the facility for a
period of no less than five years, and shall be made available to the Division upon request. At a
minimum, this log shall include:
a. Date of calibration of flow measurement device(s);
b. Date of calibration of turbidimeter;
c. Record of preventative maintenance (e.g., changing/adjusting of equipment, pump and valve
maintenance, cross connection .control, testing, inspections and cleanings, etc.; and
d. Record of all unpermitted releases of reclaimed water to surface water or land surface including
date of occurrence, estimated volume of release, cause, and corrective action taken.

6. Three copies of all effluent monitoring data and reclaimed water distribution data (as specified in
Conditions 1V.3. and IVA.) shall be submitted on Form NDMR for each PPI listed in Attachment A.
Reporting forms shall be submitted on or before the last day of the following month. If no reclaimed
water distribution activities occurred during the monitoring month, monitoring reports documenting
the absence of the activity are still required to be submitted. All effluent monitoring data shall be
submitted to the following address: Division of Water Resources, Information Processing Unit, 1617
Mail Service Center, Raleigh, North Carolina, 27699-1617. [15A NCAC 02T .0105(1)]

7. Noncompliance Notification:
The Permittee shall report by telephone to the Raleigh Regional Office, telephone number (919) 791-
4200, as soon as possible , but in no case more than 24 hours, or on the next working day following the
occurrence or first knowledge of the occurrence of any of the following:
a. Any failure resulting in a discharge of reclaimed water directly to surface waters or any unpermitted
release of reclaimed water to land surface greater than or equal to 5,000 gallons. Unpermitted
releases less than 5,000 gallons to land surface shall be documented by the Permittee in accordance
with Condition IV.5.e. but do not require Regional Office notification.
b. Any failure, due to known or unknown reasons, that renders the reclaimed water distribution line(s)
incapable of adequately transporting the reclaimed water (e.g., mechanical or electrical failures,
line blockages or breaks, etc.);
c. Any facility failure resulting in a by-pass directly to receiving surface waters;
d. Any spill or runoff of reclaimed water from the North Durham WU or the approved bulk fill
station;
e. Any time self -monitoring indicates the facilities permitted herein have gone out of compliance with
the limitations contained in this permit.
Any emergency requiring immediate reporting (e.g., discharges to surface waters, imminent failure of
a storage structure, etc.) outside normal business hours shall be reported to the Division's Emergency
Response personnel at telephone number (800) 662-7956, (800) 858-0368, or (919) 733-3300. Persons
reporting such occurrences by telephone shall also file a written report in letter form within five days
following first knowledge of the occurrence. This report shall outline the actions taken or proposed to
betaken to ensure the problem does not recur. [15A NCAC 02T ,0108(b)(1)]

V. INSPECTIONS
1. The Permittee shall provide adequate inspection and maintenance to ensure proper operation of the
reclaimed water generation and utilization facilities. [ 15A NCAC 02T .0108(b)]
2. The Permittee or their designee shall inspect the reclaimed water generation and utilization facilities to
prevent malfunction, facility deterioration and operator errors resulting in discharges, which may cause
the release of wastes to the environment, a threat to human health or a public nuisance. The Permittee
shall maintain an inspection log that includes, at a minimum, the date and time of inspection,
observations made, and any maintenance, repairs, or corrective actions taken. The Permittee shall
maintain this inspection log for a period of five years from the date of the inspection, and this log shall
be made available to the Division upon request. [15A NCAC 02T .0108(b)]
3. Any duly authorized Division representative may, upon presentation of credentials, enter and inspect
any property, premises or place on or related to the reclaimed water generation and utilization facilities
permitted herein at any reasonable time for the purpose of determining compliance with this permit;
may inspect or copy any records required to be maintained under the terms and conditions of this permit,
and may collect groundwater, surface water or leachate samples. [G.S. 143-215.1]
4. The Permittee shall inspect the utilization sites of all reclaimed water users who obtain more than
10,000 gallons per day of bulk distributed reclaimed water. Permittee inspection of these bulk
distribution reclaimed water utilization sites does not need to exceed one inspection per quarter. The
Permittee shall maintain records of these inspections for at least five years, and shall make them
available to the Division upon request. [15A NCAC 02T .0108(b)]
Vt. GENERAL CONDITIONS
1. Failure to comply with the conditions and limitations contained herein may subject the Permittee to an
enforcement action by the Division in accordance with North Carolina General Statutes 143-215.6A to
143-215.6C. [G.S. 143-215.6A to 143-215.6C]
2. This permit shall become voidable if the permitted facilities are not constructed in accordance with the
conditions of this permit, the Division approved plans and specifications, and other supporting
documentation. [15A NCAC 02T .0110]
3. This permit is effective only with respect to the nature and volume of wastes described in the permit
application, Division approved plans and specifications, and other supporting documentation. No
variances to applicable rules governing the construction or operation of the permitted facilities are
granted, unless specifically requested and approved in this permit pursuant to 15A NCAC 02T .0105(n).

4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances, which may be imposed by other jurisdictional government agencies
(e.g., local, state, and federal). Of particular concern to the Division are applicable river buffer rules in
15A NCAC 02B .0200; erosion and sedimentation control requirements in 15A NCAC Chapter 4 and
under the Division's General Permit NCG010000; any requirements pertaining to wetlands under 15A
NCAC 02B .0200 and 02H .0500; National Pollutant Discharge Elimination System (NPDES)
requirements under 15A NCAC 02H .0100, and documentation of compliance with Article 21 Part 6
of Chapter 143 of the General Statutes. [ 15A NCAC 02T .0105(c)(6)]
5. In the event the permitted facilities change ownership or the Permittee changes their name, a written
permit modification request shall be submitted to the Division. This request shall be made on official
Division forms, and shall include appropriate property ownership documentation and other supporting
documentation as necessary. The Permittee of record shall remain fully responsible for maintaining
and operating the facilities permitted herein until a permit is issued to the new owner. [15A NCAC

6. The Permittee shall retain a set of Division approved plans and specifications for the Iife of the facilities
permitted herein. [15A NCAC 02T .0108(b)(1)]
7. The Permittee shall maintain this permit until all permitted facilities herein are properly closed or
permitted under another permit issued by the appropriate permitting authority. [15A NCAC 02T
.01050)]
8. This permit is subject to revocation or unilateral modification upon 60 days notice from the Division
Director, in whole or part for the requirements listed in 15A NCAC 02T .0110. [ 15A NCAC 02T .0110]
9. Unless the Division Director grants a variance, expansion of the permitted facilities contained herein
shall not be granted if the Permittee exemplifies any of the criteria in 15A NCAC 02T .0120(b). [ 15A

10. The Permittee shall pay the annual fee within 30 days after being billed by the Division. Failure to pay
the annual fee accordingly shall be cause for the Division to revoke this permit. [15A NCAC 02T

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of a reclaimed water bulk fill station consisting of:
two variable frequency drive (VFD) pumps to maintain pressure to the distribution system; a 4-inch
overhead loading arm; a 3-inch quick connect fitting at ground level for distribution to customers; a
hypochlorite chlorination system consisting of a sodium hypochlorite bulk tank with meter pump to supply
chlorine to the pump feed system; a manual valve at the loading station with a turbidimeter, flow meter and
residual chlorine monitor; a catch basin that drains to the headworks; and all associated piping, valves,
controls, and appurtenances;
to serve the North Durham WRF, with no discharge of wastes to surface waters, pursuant to the application
received April 29, 2016, and in conformity with the project plans, specifications, and other supporting data
subsequently filed and approved by the Department of Environmental Quality and considered a part of this
permit.
This permit shall be effective from the date of issuance until November 30, 2021, shall void Permit No.
WQ0032515 issued March 7, 2008, and shall be subject to the following specified conditions and
limitations:
I. SCHEDULES
No later than six months prior to the expiration of this permit, the Permittee shall request renewal of
this permit on official Division forms. Upon receipt of the request, the Division will review the
adequacy of the facilities described therein, and if warranted, will renew the permit for such period of
time and under such conditions and limitations as it may deem appropriate. Please note Rule 15A
NCAC 02T .0105(d) requires an updated site map to be submitted with the permit renewal application.

II. PERFORMANCE STANDARDS
The subject reclaimed water facilities shall be effectively maintained and operated at all times so there
is no discharge to surface waters, nor any contravention of groundwater or surface water standards. In
the event the facilities fail to perform satisfactorily, including the creation of nuisance conditions due
to improper operation and maintenance, or failure of the utilization areas to adequately assimilate the
reclaimed water, the Permittee shall take immediate corrective actions including Division required
actions, such as the construction of additional or replacement reclaimed water distribution facilities, or
cessation of reclaimed water utilization activities. [G.S. 143-215.1, 143-213.3(a)]
2. This permit shall not relieve the Permittee of their responsibility for damages to groundwater or surface
water resulting from the operation of this facility. [ 15A NCAC 02B .0200, 02L .0100]
3. Effluent limitations for generated reclaimed water shall not exceed those specified in Attachment A.

4. Reclaimed water distributed through bulk fill stations may only be utilized for the purposes listed in
Attachment D. [G.S. 143-215.1]
5. The conjunctive use of reclaimed water permitted herein in no way negates, precludes, or invalidates
the most recent reissuance of the Permittee's NPDES permit (Permit No. NC0023841), and the
Permittee shall continue to comply with all conditions provided for therein. [G.S. 143-215.1]
6. The following shall be requirements for the reclaimed water distribution facilities:
a. All reclaimed water valves, storage facilities and outlets shall be tagged or labeled to warn the
public or employees that reclaimed water is not intended for drinking. Where appropriate, such
warning shall inform the public or employees to avoid contact with reclaimed water.
b. All reclaimed water piping, valves, outlets and other appurtenances shall be color -coded, taped or
otherwise marked to identify the source of the water as being reclaimed water.
i. All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone 522)
and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER — DO
NOT DRINK" or be installed with a purple (i.e., Pantone 522) identification tape or
polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and
repeated every three feet or less.
ii. Identification tape shall be at least three inches wide and have white or black lettering on purple
(i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER — DO NOT DRINK."
Identification tape shall be installed on reclaimed water pipelines in a visible manner, fastened
at least every 10 feet to each pipe length and run continuously the entire length of the pipe.
iii. Existing underground distribution systems retrofitted for the purpose of distributing reclaimed
water shall be taped or otherwise identified as noted above. This identification need not extend
the entire length of the distribution system, but shall be incorporated within 10 feet of crossing
any potable water supply line or sanitary sewer line.
c. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits
operation by authorized personnel only.
d. Hose bibs shall be located in locked, below grade vaults that shall be labeled as being of non -
potable quality. As an alternative to the use of locked vaults with standard hose bib services, other
locking mechanisms such as hose bibs which can only be operated by a tool may be placed above
ground and labeled as non -potable water.

7. No direct cross -connections shall be allowed betvveen reclaimed water and potable water systems,
unless such connection has been approved by the Department pursuant to 15A NCAC 18C .0406. [15A
